Suggest Treatment For Lower Back Pain

Question: About 2 weeks ago, I began to experience lower back pain, on the right side hip area. the pain was piercing, shooting right through my hip, so I saw my PC Doctor and she presribed x-ray, Muscle relaxer and P/T. I did not take the meds except for ibuprofen, and have not had X ray's yet, because, the symptoms went away. Yesterday, the pain began again and has continued. Earlier today, I had an episode where I experienced such shooting, paralyzing pain, in the same area, that I could not stand, or do anything, but freeze in place. I took 800MG ibuprofen and am applying heating pads now. The meds have helped a bit and I can walk again and move, but I am scared. What is the best way to treat these spasms, if thats what they are? What can I do now, to feel bettter, soon? Thanks

I appreciate your concern

Based on your description of the pain, it appears to be related to a urinary tract stone. Urinalysis, CBC, X ray KUB and/or ultrasound should be done as part of the workup. Anti spasmodic medication like drotaverine and NSAIDs (Diclofenac) can be administered in the E.R to relieve the spasms. Further management will depend on the cause. If a stone is located, less than 5 mm sized stones will most likely be passed spontaneously. Larger stones may require operative procedures/lithotripsy treatment. Antibiotics should be prescribed if there is evidence of UTI. If you don't have access to an ER near you now, you may look for over the counter hyoscine butylbromide or drotaverine. The ibuprofen 600 mg can be taken alongside the antispasmodics. However, I would recommend that you revisit the E.R.

Dear Dr,

Thank you, I never considered a UTI, or urinary tract stone, as the possible cause. My first diagnosis was lower back spasms. I personally thought, I might have sprained my back lifting something heavy. I'd Never had pain like that, before.

I understand completly, that you recommend I consult with a Doctor, again, but that's not doable right now. I am scheduled to be out of town, soon, and need to treat these symptoms with OTC's, for now.

What else would you recommend, as far as diet and fluids? Should I increase my fluid intake, if these symptons, are UTI, or similar.
What about applying cold or warmth to area? Is it good, or bad? It appears when I use a heating pad, it helps, and feels better.

Hi.

Yes, you should increase your fluid intake to pass a relatively diluted urine and in addition to that I would recommend adding lemon to the fluids every day till you find a doctor. Cranberry juice or extract solutions can also be of some help in dealing with the UTI. Other diet changes would only be relevant if the diagnosis is confirmed. You can apply the warm heating pads as hot fomentation can help in symptomatic relief from the pain. I have already stated the OTC medication that you need to take in the previous reply.
