Suggest Treatment For Lower Back Pain And Frequent Urination

Question: Hello, about 18months ago i started having lower back pain and more frequent urination. Until one night about 4 months later i felt like i needed to pee a lot and could not. I had to stand over the toilet and force a bit out drip by drip. I had urine culture which was negative but doc gave me copra in case i had Prostatis. I had prostate exam the following week and everything was ok. But then an now for last 13 months or so i have constant tingling feeling in my urethra and abdominal pain, sometimes also lower back stiffness. All my bacterial tests have came back negative, MRI shows just slight herniated disc 2 of them but not touching any nerves. Also cystoscopy shown slight tight bladder neck. I have been on flomax and Mirabegron, just keeps signals down slightly but feeling always there. My bladder feels knotted a little. Can you please help i am so stuck? Someone suggested by be Pelvic Floor Dysfunction in male? My urologist thinks it is sensory urgency in bladder but i have had this over a year!

I would advise you do get a Uroflometry done to assess what kind of bladder dysfunction you have.

Do perform Kegel's exercises for 5 days a week to improve Pelvic floor muscles.

Get tested for Diabetes if you haven't already.

I have had uroflow report and the findings where:

maximum flow rate of 13mls/sec on a void volume of 135mls and insignificant residual
i had my pelvic floor looked at and they said it was hypertonic, hence why i get some pelvic pains. But main issue is constant feeling tingle in urethra and urge to urinate. This is even worse after bowel movement. After bowel movement i have stronger urge to urinate for many hours.

Since you have hypertonic pelvic floor muscles, they contract in an abnormal rhythm, out of sync.

This is the reason for your urinary disturbance. After eating, the involuntary muscles tend to contract more due to parasympathetic activation which is why the disturbance is more during that time.

Do perform Kegel maneuver. Hey am opinion by a Gastroenterologist regarding the need for Muscle relaxant in your case which can help you.

Ok so it is possible I have just muscle disturbance not Cystisis. I was told doing kegel would be bad for hypertonic and just to relax? Will this go eventually? And can it cause tingle in urethra?

Needs relaxation methods too. Urethritis can cause a tingle also

Cystitis would be picked up on the scan. Besides you would also have fever, burning micturition and lower abdominal pain.

Kegel's will help in maintaining good resting tone of the muscle but yes, you will need to discuss about relaxation methods with your Physician. This problem will be resolved as soon as relaxation techniques are commenced.

Tingle in the urethra could be due to the hypertonia and also due to urethritis. Drink plenty of water. You can drink Syrup Alkaline citrate 3 teaspoons thrice a day, to reduce the urine acidity and reduce tingling as well.
