Suggest Treatment For Low EGFR And Mild Anemia

Question: My eGFR was 57; previously it was above 60 (in 89-92 range). Creatine was 1.35. My Doctor says that because she saw no albumin or proteinuria, she suspects dehydration (and I probably DON'T drink enough water). She recommended I rehydrate and she'll test again in 3 months. Also I was mildly anemic-my Doctor feels this is nutritionally caused because I've avoided all meat and fish for months. She instructed that I reintroduce meat into my diet. I've been type 2 diabetic since 1999 and have HTN which is controlled. I've been walking an hour each day and working with light weights since January. Do you agree with my Doctor's present recommendations to address eGFR and mild anemia?

Glomerular filtration rate (GFR) is a test used to check how well the kidneys are working. Specifically, it estimates how much blood passes through the glomeruli each minute. Glomeruli are the tiny filters in the kidneys that filter waste from the blood.

Your GFR is slightly lower. But do not worry. Your doctor is right. Repeat the investigations after 3 months. You are diabetic and use Benazipril , both can lead to a decreased GFR over long term. But Benazipril which is an ACE inhibitor should still be continued despite the decreased GFR as it has more benefits in the long term by reducing intra glomerular hypertension and preventing further kidney damage.

Anemia is suggested in males with Hb levels less than 13.5 g/dL and in females with Hb levels less than 12.5 g/dL. Higher values are anticipated in individuals living in altitudes significantly above sea level. Variations to these numbers exist, too. Yes you should include meat in your diet and a peripheral smear would be indicated if your Hb remains persistently low despite diet modification and iron and folic acid supplementation.
