Suggest Treatment For Low Progesterone Levels While Having ICSI/IVF

Question: Dear Dr. Greetings of the day.
I have done two ICSI/IVF. In 1st time, one embryo was transplanted. In the 2nd time, after one yr, two embryos were transplanted. In 2nd time my wife got pregnant. but the embryo could not settle down in the uterus due to reduction of Progesterone Hormone. In 1st case also, her progesterone hormone dropped down. How to solve this problem now? Need ur kind help doctor. Thanks.

Hi,

I understand your concern about reduced progesterone leading to abortion.

This is called luteal phase defect where in progesterone is reduced. Hence progesterone supplementation is required. I suggest you take progesterone hormone injections and tablets in next pregnancy to prevent progesterone hormone from reducing.

Dear Dr,

progesterone supplementation (through vagina) was given for three weeks after the embryos were implanted.

The 1st test result showed progesterone level was 39. But the doctor wanted it more than 40. She increased the doze of progesterone supplementation.

But after 9 days, 2nd test result showed progesterone level gone down to 22.
Doctor again increased the doze, three times a day.

3rd test after one wk, showed the level remained at 22.
But in all 3 tests, the baby hormone, was increasing. In last test the baby hormone reached up to more than 2000.

The Doctor, initially thought that may be she became pregnant by normal intercourse (since we did sex, just few days before the implantation). But, later they concluded that both the embryos disappeared. The Ultrasound resulted no beat of baby.

After that, they advised to stop progesterone supplementation.

Now, patient has crossed 5 months or so after IVF failed.
What is our next step?
We did this IVF at Fertility First. Still we have 6 embryos preserved.
My wife doesn't want to go to this process again, becoz of the physical and mental stress, which she fears much.
Will it be possible to get her pregnant normally?
How she can overcome this progesterone reduction problem? If we do IVF again, how can we deal with this problem?

Thank u doctor. Need ur valuable advice to continue our treatment, either at a new clinic or at the previous clinic.

Progesterone may not be the issue here

Hi,

Though progesterone levels were less, i dont think the Failure was due to progesterone.

Progesterone was given in good quantity.

I presume there was problem with embryo quality as it is normal to have embryos which are not healthy especially at 42 years age. Though embryos look normal at outside, there are some chromosomal defect which make embry unviable and abort earlier. That seems to be more possible reason for such early stopping of beta hCG rise and not progesterone values.

I suggest you also take injectable progesterone next cycle along with progesterone suppositories. Injectable progesterones help in bringing up the blood levels faster.
