Suggest Treatment For Loose Motions And Heavy Stomach Ache
Question: I am suffering with loose motions and heavy stomach ache at right side bottom of stomach and right back also. I was diagnosed with gall bladder stones and fatty liver grade 2 four months back. I am undergoing homoeo and ayurveda treatment for it but since yesterday I am feeling pain in right bottom stomach and back with loose motions. Please advice.

How are you? My name is Dr Suresh Raghavaiah. I am a liver and Pancreas specialist and I hope to answer your question today.
I am sorry to hear about your diagnosis.
Regarding your complaints, one possibility is pancreatitis, given you typical history of pain in the stomach and back, especially in someone who has gall stones. Of the gall stones are small, one of the stone usually slips down into the bile ducts and causes blockage of the ducts. This leads to "back up" of all the bile and pancreatic juice and leads to jaundice and pancreatitis. This could become serious very soon. I advise you to visit your neared physician / surgeon to get it evaluated quickly You will need blood tests including Liver function tests, Serum Amylase and lipase levels. You will also need an Ultrasonography of the abdomen and maybe a CT scan of the abdomen as well.
One more possibility is kidney stones, Ultrasound scan will help you to diagnosis.
Please let me know where you are from so I can suggest any hospital close by which specialises in treating your condition. I am not an expert on home or ayurvedic treatment. So I cannot comment on continuing that. But please do go see a Allopthic physician or surgeon at the earliest.

Hello Dr XXXXXXX Thanks for your reply. I did visited Gastroenterologist twice 2 months back and also as a follow up after 1 month. I had US scan and CT scan too. The findings were gall stones measuring 8.8 mm and liver measures 15.6cms mildly enlarged in size. diffusely reduced attenuation of liver parenchyma. normal in enhancement pattern no focal lesions. evidence of enlarged 12 mm mesenteric lymphnode noted. long redundant appendix, normal in diameter no periappendiceal fat stranding noted. mild hepatomegaly with fatty changes, cholelithiasis, enlarged 12 mm mesenteric lymphnode were measure impressions.doctor did advised tab QROZ before breakfast for 1 month and tab GOLBI 300 for 2 months 2 times and review after 2 months. I have not used the said tabs but took homoeo. Please suggest shall i take the said tabs now?i stay at kachiguda, XXXXXXX

There is nothing on the CT scan which would warrant a surgical intervention at present. Even though the stone is present, there is no evidence that it is affecting the gall bladder in any way. The only problem it could potentially cause is if it slips down into the bile duct. But as of the current scan, there is no need for it to be removed. I do think that you should start with the tablets prescibed by your gastro-enterologist. XXXXXXX has a number of excellent medical centers which can cater to your problem, but as you have already visited a gastro-enterologist, that should suffice.
Please do take the prescribed tablets. If the tablets do not give you any relief, then we might need to consider a laparoscopic cholecystectomy to get rid of the possible biliary colic.

Thank you so much once again Doctor for your reply and patience. Is there any problem with long redundant appendix and enlarged 12 mm mesenteric lymphnode? the surgery you prescribed is removal of gall bladder or removal of only stones? I learnt that removal of gall bladder will have many after effects? removal of gall bladder is only only way to get rid of gall stones or is there any evidences in history so far that without surgery a patient has got rid of it?

I would not worry about the appendix unless it starts to trouble you. Yes there is a theoretical risk that a long appendix might be more prone to infection, but if it has not done so in so many years, i would very much doubt if it is going to trouble you in the later years. I am "intrigued" by the 12mm mesenteric node...but not overly concerned by it. Unless you develop any other symptoms like persistant fever and pain abdomen, i would not worry about it.
Regarding removal of the gall bladder, I must disagree that it will have effects after the surgery. It is one of the most commonly performed operation in the world. It is very safely accomplished at many surgical centers by the laparoscopic technique and the average stay in the hospital is one day!!. There are occasional reports of pain abdomen persisting after cholecystectomy and also occasional reports of diarrhoea occuring. But these are a very minority of the patients and should no way affect your decision to undergo a cholecystectomy. Unfortunately, once stones have been formed, it is impossible for medications to treat the same and you will need a surgery in case you become symptomatic.
