Suggest Treatment For Lethargy And Weight Loss When Suffering Form High BP And Diabetes

Question: Hi

Its related to my mother, She is around 50 years old. She has undergone 3 operations:-

1. Her Gall Bladder has been removed.
2. Her Uterus has been removed because there was fibroid in it.
3. Then she underwent surgery for Hernia.

She is diagnosed with following diseases:-

1. High BP - She is regularly taking Amlopress - AT

2. Diabetic - She regularly takes Amryl-1mg, sometimes takes Amway Glucose Health, Takes some home remedies for Diabetes, but does not have time for exercise or yoga. Her sugar levels are in normal range but sometimes even if she takes all the medicines they keep on varying.

3. Thyroid - She takes a medicine 25 mg regularly in morning, here TSH test is normal now but still she feels weak and area near her neck has become very thin.

4. Indigestion and Gastic issues:- She has lot of gastic troubles due to which many a times she has sever headaches in morning. To relieve herself she needs to vomit and only after that any pain killer works. Thats the biggest issue she is facing as the headache remains till night even if she vomits or takes any pain killer. she keeps on taking ant acid medicines like Zintac, Rentac or trying home remedies which sometimes work but most of the times they fail.

Now the problems are:-

1. She takes all her medicines regularly but these days her health is deteriorating. She feels week, unenergic, feels lathargic at times and area near her neck is getting thin. She is losing weight and we are not able to understand that when all the tests are normal what else we need to take care of.

Can you please recommend what other tests she need to unedrgo to find the root cause of her health issues. (she has already done the tests like Thyroid-TSH, Diabetic test, HBA1C and BP monitoring)

Hi,

Many a times we get to see multiple symptoms spread across the body in a few patients. All the complaints may or may not be related to each other. Your mother's symptoms may be related to diabetes - thyroid condition - gastritis - inability to sleep well. However it would be best if all these issues are addressed by individual specialist in the beginning. To explain further I shall break her symptoms to most concerning.

1. You report that she has severe headache early in the morning with repeated vomiting. This is consistent with chronic headache. Perhaps lack of sufficient sleep may have triggered it. Persistent and regular headache can explain her being less energetic. I recommend that you visit a neurologist and solve regular headaches. There are prophylactic medicines that help reduce the severity and frequency of headaches.

2. Severe gastritis may be due to dietary and life style issues. Upper GI endoscopy can be a worthy investigations to rule out other causes. Regular diet, less spicy food, restricted coffee, regular sleep and small amount of daily exercise are essential components of treatment. You can contact a gastroenterologist for endoscopy.

3. Weakness, easy fatiguability, unenergetic and drowsiness may very well be due to lack of sleep and regular headache. Other causes of anemia and nutritional deficiency can be ruled out by the neurologist or her current physician.

4. Further hormonal disturbances secondary to menopause / perimenopause can also lead to those symptoms. Most often menopause related complaints resolve over a period of time.

5. I don't think her neck becoming thin is a significant concern. It may be due to dehydration secondary to repeated vomiting / nutritional deficiency and not from serious organic condition. If she has not lost significant weight, I wouldn't be worried about the emaciation.

In the nutshell, there is significant interaction between all her complaints. You can visit different specialist to rectify it.

PS: I presume her recent thyroid functions and blood sugars are normal. If she hasn't been tested recently say in the last 3-6 months, it is prudent to repeat the tests.

Diet is only a supplemental treatment. It is important to address all her current medical conditions with appropriate treatment. Therefore I insists she consults her doctors and get her conditions treated well. I would like her doctors to first treat her headaches and lack of sleep. They seem to be the cause for most of her deterioration. So consult a neurologist.

As far as diet is concerned, it will help if she can
1. Reduce the amount of spices. Hot and spicy food can worsen gastritis leading to the cycle.
2. Avoid fasting and skipping meals.
3. Reduce the amount of salt, high calorie and high fatty food.
4. Avoid caffeine and other aerated beverages.
5. Watch out for any precursor food that initiates migraine.
6. Include plenty of fibres, fruits and vegetables.
7. Drink plenty of water and fluids.
8. It will also help if she adds moderate exercise to her life style.

These seem to be generic advice. More details on her diet can be obtained through a dietician based on her clinical illness.
