Question: i am having unexplained leg pain with swelling in left inner elbow lymph node and arm. Pain in legs and hips increases at rest, interrupting sleep. What can this be?

Hi Dr.,
The pain began suddenly on Tuesday afternoon after returning from a road trip with 4 hour car ride. There is no swelling in my legs. They look normal, good pulses and sensation. No bruising to legs. The pain has persisted for the last 2 nights waking me up. Last night I woke at 3:30. Pain relieved by a warm bath at 5:30. Then returned once out of bath. Tylenol taken at 6:30 and pain subsided.
My left inner elbow has not sustained any injury. The pain in my elbow began approx 4 months ago as a noticeable pressure in my left inner elbow and then a painful swollen lump approx 2inches in diameter became evident. The lump increases and decreases in size with varying degrees of pressure and pain. I think it may be a swollen lymph node. I noticed it the same time I noticed a very small dry patch on my right cheek. I went to a dermatologist who biopsied the skin after months of other treatments that did not help. The biopsy revealed abnormal, precancerous skin cells (actinic keratosis). I am on my 2nd round of using Zyclara, topical chemo, for a 2 week period. I have to apply it to my cheek for 2 more nights. I was thinking about calling the dermatologist and asking if my leg symptoms could be linked to the Zyclara. My inner elbow pain, swelling and pressure predates the Zyclara but aligns with discovering the dry patch on my face. I have also noticed my left arm is slightly larger and edematous compared to the right one. I have thin arms so the changes are very evident to me. Physically, I am very fit and healthy looking so I think most people tend to think nothing could be wrong with me. For example, it was at least 4 visits to the dermatologist before she would do a biopsy of my skin and even then she was reluctant b/c it might scar but I was determined something was not right. It has been expensive since I do not have insurance. Now, with my leg pain and the persistent swelling in my elbow, I am afraid of the additional costs to figure out what is wrong but I know I will probably have to bite the bullet and do something soon since my elbow symptoms are persisting and worsening. I just don't know where to begin. Should I go to an instant clinic since I do not have a regular doctor and do not have insurance?

Hello

Thank you for gettig back with the answers. I have gone through them. Your symptoms need further workup at a proper tertiary care hospital. there are certain conditions which cause generalized lymphadenoptahy that we need to rule out. Workup will include baseline investiagtions like CBC, peripheral film, USG-Abdomen & USG- swelling. Results of the labs will help to proceed further. Secondly, pain in legs could be muscular spasm. Zyclara has some generalized side effects which are present in you as well but as you mentioned that you only need to apply for 2 more nights so hopefully when you will stop using it then your symptoms will improve. I would suggest you following things meanwhile:

- naproxen thrice a day for 3 days.
- take hot water in a rubber bottle and place it at the site of pain. It will give soothing effect.


Get an appointment for a "consultant Medical Specialist" for further workup after stoping Zyclara.
