Question: My husband is an 81 yr. old diabetic with lower leg edema from congestive heart failure. He has developed extreme swelling in his scrotum and foreskin that has lasted almost a week. He has a few hard fluid filled lumps in his scrotum. What could be causing this and how can it be treated? He does not want to go to the doctor and as we live in a rural area, the trip would be quite uncomfortable for him with the swelling.

I read your question and i understand your concern, At age of 81 with diabetes mellitus and congestive heart failure, edema is inevitable and needs compliance with supportive care of diuretics and nitrates to help it out. With this scrotal swelling for a week, that needs to be examined. It may be scrotal edema just like pedal edema (legs) and a continuation of heart failure or it could be a local cause . Any how the scrota needs to be examined and if possible had an ultrasound of. Only then one can comment on the cause and treatment. Try scrotal support like tight underwear and see if it helps. But i am afraid he has to go to the followup.
