Brief Answer:
genetic or hormonal.

Detailed Answer:
Thanks for the query.
I can understand your concern.
You need to do hormonal evaluation like thyroid hormone level.
Basal Metabolic rate also determines the weight gain or weight loss.
There may be genetic causes like if parents are thin child may also be thin.
Do not give her outside food like chips, toast, biscuits.
Give more home made food.
Exercise daily.
Try appetite stimulant like cyproheptadin along with iron.
Hope i helped you.

Brief Answer:
you can continue alfalfa.

Detailed Answer:
Thanks for the follow up query.
Generally homeopathic medicine does not have any side effects.
Giving allopathic medicine along with homeaopathic medicine , will not cause any effect to allopathic medicine but the efficacy of homeopathic medicine may be hampered.
So see your homeopathic doctor to confirm whether you should continue with alfalfa.
To name some medicine, i would give my patient syp. cypon 7.5 ml two times a day
before food and syp. tonoferon- pediatric 7.5 ml two times a day after food.
Continue both the medicine for 3 months atleast , you will definitely get the results.
Hope i helped you.
Happy to help you in future.
