Brief Answer:
Possible STD

Detailed Answer:
Hi and welcome.

If there was recent intercourse then STD should be always ruled out, especially herpes.

In this case, it would be good to do tests (complete STD panel check-up). Other causes include epididymitis, fungal infections, or anal pruritus and in this case, mild steroid and anti fungal cream may be beneficial, such as Nystatin.

Feel free to ask further.
Wish you good health.

Brief Answer:
Follow-up

Detailed Answer:
Hi again,

Some STDs may occur even after this time but this is rare. it may then be epididimitis or parasitic perianal contamination and it would be good to get a testicular ultrasound scan and parasitic testing of feces.
