Question: Hi, I have developed red colour rashes on my penis top after we pull the foreskin down like 2cm diameter area where skin is looking like peeling off. This is at 2 per 3 places and makes me feel itchy and bit of swelling and purplish colour is there on the edge of penis top.

What can be the cause and what medicine shall I apply to ease out the situation.

Brief Answer:
Appears to be fungal infection.

Detailed Answer:
Hi
Thanks for your query at HCM.
I understand your concern and situation.

As per your history it appears that it is some fungal infection. You can apply triple action cream on the area thrice daily for about 2 weeks. If you had history of unprotected intercourse you might require panel of investigation to rule out STD. If no history then nothing to worry about. Maintain genital hygiene and refrain from the sexual activities for 2-3 weeks. Avoid usage of latex condom as it can aggravate the lesion.

Hope I answered your query

Brief Answer:
Over the counter topical agents

Detailed Answer:
Hi

I understand your problem. If you have no facilities for testing then i would suggest you to apply triple action cream(Anti fungal, Antibacterial and steroid) which are available as over-the-counter and apply it thrice daily regularly and maintaining the hygiene at the same time. drink plenty of water. Usually the cause of such fungal infections could be related to hygiene and they usually cause itching.

Take care

Thank you for prompt responses thus far.

What is the problem by your assessment looking at the uploaded pictures.

I also read that Clobenate GM cream excess use also causes rashes, and have been using same for on the foreskin for last 1 month or so. Shall I stop applying this cream.

I also have been given another cream called Sebrasten by local physician, is that ok to apply or will you suggest a specific triple action cream.

Doc am really worried.

Just going back, am having symptoms in some shape form for since mid Jun, when I suddenly developed teeth marks on my tongue and for 2 months my tongue had an inflamed feeling, and burning feeling which was changing position every day, however after taking SM fibro tablet and B complex capsule plus wearing teeth cap, the teeth marks have all gone. Then 1.5 months post the teeth marks on 1 Aug, I suddenly started developed boil on the anus lining, which did'nt pain or discharged or itched and with homeopathy this boil went away in a week. And then suddenly in early Sept I developed itching and bit of red bump under foreskin, which has still not gone. Lastly from mid Sept onwards have developed Staph infection led boils on both my shoulders and upper back, these boils ultimately had small pus white mark on top with pain but no itching.

Thought u should know the last 3.5 months history before advising.

Look forward to hearing from you.

Brief Answer:
You are taking appropriate treatment.

Detailed Answer:
Hi
Welcome back.

I read your condition. Yes your doctor has given you antifungal cream clotrimazole. It is good medicine and will surely bring relief. As far as your other problems are concerned of recurring boils. I would suggest you to get your blood sugar and HbA1C levels monitored. If levels are within normal limits nothing to worry about. Keep your general hygiene maintained. Also get blood haemogram especially DLC levels checked. Follow up with reports. Do not worry you will be fine soon. Take treatment as advised. Yes you can stop clobenate GM.

Thank You doctor for confirming the medicine.

It really has brought down the itching sensation, though after 2.5 days of application twice a day, I find that skin of penis top has turned red and more red marks are there on the penis top or around the rim. It seems swollen at time though itching has gone down considerably.

There is no pain or discharge.

Shall I continue with Sebrasten application or stop.

Shall I see a doctor or wait for 2-3 more days and then visit the doctor.

I had done my random sugar and haemogram test just month before and all was ok. Even thyroid was also ok.

Look forward to your advice.

Brief Answer:
Yes tests recommended.

Detailed Answer:
Hi

Yes even if you are feeling better or your lesion improve still I would suggest you to get evaluation done for STD. These include serological, molecular for HSV, Chlamydia, Gonorrhea. Get VDRL test to rule out syphilis. I would even recommend as per your history a 4 th generation ELISA test for HIV.
You must not worry to take the tests as they are only precautionary.
Do not hesitate to ask any query. We are here to help you.
As far as your present symptoms they do not hint towards any specific infection.
Remember you have to combat anxiety. You can practice meditation and get engaged in work. Have trust in your God almighty as well.

Hi Doc, got to know one new thing, hence writing.

Last female who I came in contact with had a hormonal acne issue, for this she was using feminine hormones (contraceptive called Diana). Can my symptoms be attributed to contact with her...

If yes then what can be possible treatment.

Thanks Doctor, this further calms me down.

Let me ask some other of my other clarification points. sorry if these are very explicit but want to clear my doubts :

1. Can accidental drinking of breast milk (limited quantity) cause the symptoms I had? I had an accidental drinking in July. however my teeth marks on tongue was there from Jun onwards.

2. Can saliva spit on uncovered penis cause any of these symptoms - had from someone during late Mar but these symptoms started from middle of Jun.

Sorry if am coming one after the other, but it is taking some courage to spill out everything and due to mental stress I don't remember everything in one go.

Look forward to your response.

Brief Answer:
Hypothetical risk only.

Detailed Answer:
Hi

I understand your concern. See in adults breast-milk only a hypothetical risk of transmission is present. Now this will depend on the amount of virus in the infected milk, quantity ingested, presence of ulcers or oral lesion in the oral mucous membranes. But I must emphasize that the risk of an adult contracting HIV from nipple noshing would be extremely low. Similar is the case with saliva spit on penis. But to clear doubt I would suggest HIV testing. You are always welcome to ask queries. We are here to help you. Do not hesitate
