Suggest Treatment For Irregular Periods And Brown Discharge

Question: Dear Doctor,
Since Dec'14 my wife had irregular periods. I consulted on this portal to Dr XXXXXXX .who diagnosed as "Hormonal Imbalance"
I went forward and consulted local Gynae Dr XXXXXXX Kant at Asian Hospital. She strongly recommended D&C.Same was carried out on 13/02/15. Biopsy was negative for malignancy.
Post D&C medicines the prescribed medicines including "Modus" are as per attached prescriptions. Brown discharge continued quite often.
But after three months, during mid May'15, some red spots were located on the skin- mainly on thighs, arms, waist.
Patient was advised to consult the Skin specialist- on the same day.
I was told that this was this was possible reaction of the gynae medication. Skin biopsy showed "Lymphoplasmacytic vasculitis"
Subsequently ANA Test, Blood Test for Liver , Kidney, etc were done. All reports are attached. Medicines prescribed were Zempred, Pan 40, Psorid 50 etc.
My QUESTION:
DELAYED PERIOD : But LMP being 13/06/15, it has already crossed 45 days but there is no sign of Period. Is it related to some SIDE REACTION to medication ? Occasionally there is mild pain in lowr abdomen and breast. Gynae has diagnosed there is no lump in the breast.
RISE IN EYE PRESSURE : She is under treatment for elevated Eye pressure for last one year. Gluocoma is ruled out and in the month of XXXXXXX the pressure was 21/22. But in July it was seen as 32/32. Doctor is also surprised.
Could you please guide after assessment of all the symptoms narrated above.

Thanks for writing to us with your health concern.
I have gone through all the reports and the information that you have provided.
There are some separate issues here.
1. Irregular cycles - Yes, some of the medications being prescribed for her skin condition can lead to irregular periods.
However, at 49, naturally too, periods will get irregular as she is most definitely nearing the menopausal age.
So if irregular cycles is the only issue, do not worry.
Cyclical progesterone therapy should correct it.
If she is still having heavy periods though, then there are 2 options left - Mirena or surgical hysterectomy ( if the symptoms are very troublesome ).
In my opinion, menopause will set in within some time, until then she can manage with medications.
2. Eye pressure
The pressure is really high, and it should now be treated, as there is risk of optic nerve damage.
Please talk to your opthalmologist regarding starting treatment and regular monitoring to watch for any eye damage.
3. Lymphocytic vasculitis
She is already under proper treatment for a dermatologist for the same.

Dear Doctor,

I missed to say that patient is taking BP tablet - Metosartin 50.

I am attaching Eye Prescriptions and EYE BALL Image.- which has ruled out Leucomia. Yes she has cataract in RE.

Please advise what could have made the pressure increase.

Well, hypertension is the reason for the cataracts
Raised intrao cular pressure in absence of cataract has no known reason.
Raised BP maybe a reason but it is not proved.
It is a syndrome without a specific reason.
What is important is that it should be treated before it damages the eye ball and the optic nerve.

Dear Doctor,

Good Morning !

I am curious to know if all symptoms (Gynae, Skin and Eye) are somehow interrelated ?

Well Vitamin D deficiency is noticed..it is "8" against normal "30"- a minimum. What influence has these on all symptoms. How to rapidly make up this deficiency (Injection/ tablets ?). Will Vitamin D have any further side effect ?

Kindly help me.

Hello again.
The skin condition that she helps is suggestive of an auto immune disorder.
Such disorders generally target multiple organs , so some connection is possible, but it cannot be proved right now.
And the gynec condition is more related to her age and peri menopausal status.
Yes, she does need to take vitamin D supplements.
Sachets of Vitamin D ( D 360, Mashyne ) etc need to be taken once a week for the first 4 weeks, and then daily calcium plus vitamin D tablets ( gemcal - D3 ) should be taken for atleast 3 months.
No, there will no side effects of Vitamin D therapy.
However, these are prescription medications so it is best to start after consulting your doctor.
