Brief Answer:
Two possibilities.

Detailed Answer:
1. Internal organ injury at the time of fall.
2.Incidental finding of deranged LFT - liver function test.

Welcome to health care magic.

1.The Ultrasound which was done immediately after the fall can many times miss the internal organ injury immediately after trauma.
2.There was internal collection / bleed as you mentioned - that suggest there was some injury internally.
3.The Liver function changes can be incidental findings - as many of them do have deranged but once detected you need to get treated the cause.
4. Paracetamol is minimized because it will increase the load on the liver and delay the healing process. You can take alternative if its not fever.
5. In this case a CT abdomen with contrast will be ideal to evaluate any internal cause and liver pathologies in more detail.

Good luck.
