Suggest Treatment For Intermittent Pain In The Left Rib Cage

Question: Over the last month I have had intermittent pain in my left back below my rib cage, where my left kidney is located. The pain has been dull, and has mostly gone away after a few minutes. Tonight the pain has come back, but it is not going away. It is a dull pain that comes in waves. My urine is clear. I have been drinking a lot of water this evening in the event that the pain is a kidney stone. My medications: simvastatin (20mg), amlodopine besylate (5mg) and oxcarbazepine (600mg 2x/day). There is no blood in my urine. I have taken 1,000 mg of acetaminophen. Should I try to go to sleep (it is 11:00 p.m. in XXXXXXX Beach)? Should I stay up and keep on drinking water? Should I go to the ER? What do you advise?

I have gone through your query and do understand your concern.
There are two possibilities that you can discuss with your local doctor. I'll briefly mention them.

1. Muscle spasm and
2. Kidney stone .

For muscle spasm : You should take NSAID like Aceclofenac with muscle relaxants like Chlorzoxazone/ Tizanidine twice daily for 3-5 days based on the response. You can ask your local doctor for prescription of this agent. Along with this hot compression and analgesic gel or spray will be beneficial.

For kidney stones: first of all it need to be confirmed, for this purpose I would have suggested you for KUB X-ray and ultrasound abdomen, based on the investigation report one can decide on the management plan. So for this kindly visit urologist for further help.

Other possibilities is simvastatin induced myopathy. It iss usually associated with generalised muscle weakness and aches, even though less likely, have to be ruled out in the absence of above mentioned possibilities.

If the pain is manageable you can take rest and see doctor tomorrow.
