Suggest Treatment For Infected Cyst On Back

Question: I have what I thought was an infected cyst or lipoma on my back hurts when I lean back in chair ...although not as much as a few days ago but has been there hoe about a month. It's raised enough that you can see it through my shirt. Anyway I had an appt with Endo today so I asked if she could look at it . The PA thought that it might be some kind of insect bite because there is a black dot. Good news I don't need to see the surgeon to have it removed but Should I be concerned of a tick?

I don't know how to up load picture. Some joint stiffness night sweating only this mass do insect bites usually last this long. It is starting to itch more so that may be a good sign right?

Yes, you are right, usually, insect bites won't last for this long (1 month). At this point, you should have had other symptoms if Lyme disease.

According to your description, it might be a possible:
- skin infection (abscess)
- infected lipoma
- insect bite

However, I need to have a look at the skin mass first before running final conclusions.

Hi back,

Thank you for sending in the pictures although not very clear.

Judging from what I have seen, it seems to be an infected lipoma. The treatment is surgical removal. Oral antibiotics can be used as prophylaxis, however, they cannot get into the capsule of lipoma to fight infection.

You might need drainage of pus if it is present. I advise to get your blood sugar levels checked and do not squeeze or scratch the mass until you see your doctor.

You can discuss with your doctor if you need to take a material to run biopsy. To my opinion, it is not necessary unless anything unusual is seen during surgical removal of it.

Please do not try any remedy on your own; let is do to the surgeon. Once again, do not get scared as the it is the simplest intervention, but need regular follow up.

Thank you for your quick reply. I had made an appointment with surgeon but when the Physicians Assistant said it may be an insect bite because black dot. I canceled appointment. Don't want to look stupid or have surgeon schedule for surgery an it's only a bite. But with my history I think your right. And my non fasting glucose was a tad high. Should I ask for a regular glucose test as I thought my shakiness was sugar or thyroid related. I will be rating excellent after next reply letting you know cause sometimes I try to do the rating and it doesn't go through.

It's my pleasure to assist you and provide a second opinion.

Now, from what I have seen, the skin at your back is covered with black dots, most probably due to pigmentation. So, my judgement was that yours is not related to insect bite although the picture was not clear.

It is not a stupid thing to consult with a surgeon even if not a case of lipoma (although it has the signs of infected lipoma). At least, you will exclude surgical problems.

Next, usually skin infections are associated with impaired glucose levels. That's why I have asked to run diabetes testing, which include:
- fasting blood glucose
- oral glucose tolerance test
- HbA1C testing
- complete blood work (to exclude anemia)

With regards to shakiness you have experienced, you are right, it might be related to :
- thyroid problems
- low blood sugar levels (especially if you skip meals)
- hypotension sometimes caused by
- dehydration (if it is hot at the place you live and you sweat a lot).

If I was your caring doctor would ask to run these tests for further evaluation of your health status and to determine the right diagnosis for proper treatment. They are not serious problems, but need their proper medical attention.

My opinion is based only on information provided by you without a proper physical examination. That's why I am asking to discuss them with your doctor and run proper examinations.
