Suggest Treatment For Increased Leukocytes In Semen

Question: Hi,
I had semen culture test last month showing 14-16 hpf pus cells in semen. Also has left testicular pain. USG suggested right inguinal node enlargement. Dr prescribed antibiotics for 14 days.
I discontinued antibiotics in middle because the semen report was normal also abdomen USG was normal.(a week ago).
Latest (2 days back) semen analysis suggests 25-30 pus cells but no testicular pain. Having back ache since last week. How serious the disease is? Please help.

I have gone through your query and reports thoroughly.

Increased WBCs in semen or leukocytospermia is condition which has been studied widely but received conflicting conclusions.

I personally believe that it must be treated with antibiotics as it can affect sperm count and motility (depicted by variation in your reports).

Although not a very serious issue, but you should continue the antibiotics as directed by your doctor. The swollen inguinal nodes suggest infection/inflammation which be treated with adequate duration of antibiotic treatment.

Doxycycline and cotrimoxazole are first line agents for this condition. Vitamin E supplements are also found to be useful.

I do not feelback pain to be related to this. Ibuprofen will help in this.

So, kindly follow your doctor's advice.
Do not worry as the condition is completely curable.

As my latest (on Monday) report shows 25-30 HPF WBC in semen, I undergone semen culture test (on wednesday) which came negative today. I re-started antibiotics on Tuesday evening.

What could be the possible reason for high WBC but no bacteria in semen? I do not have any urological problems now.

My other symptoms are:
Back pain when lying or sitting
Mild pain in arms and leg
Gas
Anorexia
Dry mouth
Mild diarrhoea
Confusion
Depression

Yes, from few days I noticed lower middle part of my body (from hips to legs) become thinner and belly becomes larger. My weight (93-94 Kgs) remains same for almost last one year.

How long it will take inguinal lymph node to become normal in size?

Sorry for the late response.

There are some conditions like non-bacterial prostatits which can cause inflammation. This can raise WBCs in semen.
Although you have no urological symptoms, antibiotics are still indicated to prevent secondary infections.
Ibuprofen will reduce the inflammation.

Your other symptoms like dry mouth, anorexia, confusion, mild pains, gas can be a part of anxiety-depression.
Sibce when are you having these symptoms?
Do you feel that these symptoms developed after you got your semen reports?
Practice relaxation exercises like deep breathing and meditation.
Drink lots of water.

Back pain can be a symptom of prostatitis or more commonly due to a bad posture.
Ibuprofen will help in this as well.

You feel that your lower-middle body is thinner than your belly. I think you are excessively anxious about the reports. Thats why you are feeling this.
Please do not worry. You will be fine.

Lymphnodes will disappear in 2-3 weeks.

Dear Doctor,

Thanks for elaborated answers.

I visited a spine surgeon on Friday. He advised me to go for a MRI of spinal cord. The Results of MRI is as below:
Diffuse posterior and left forminal disc bulging at L4-L5 level with mild facetal arthropathy causing on left exiting nerve at foraminal level.

Heat therapy gives immense relief from back pain.

I yet to visit the spine surgeon for further advise. Meanwhile there is mild pain started in my left testicle again. Thus urologist told me there must be atypical bacterial infection which do not identify in routine tests and he prescribed me doxycyclin for 15 days with tramadol.

I feel very anxious and confused since one week. I could not sleep properly at night. Anorexia must be a symptom of it. I also got some symptoms of viral infection like discomfort in throat, phlegm, voice change, mild nasal discomfort etc.

Please tell me what should i do? Should I visit another urologist or continue with it?

Well, you got the reason for your back ache in your MRI report. You spine surgeon will help you to deal with it.

Testicular pain may be due to some infection.
I think your urologist is right. Yiu must continue with the abtibiitics which he has prescribed.
He must have given tramadol for pain relief. This drug can cause anxiety and confusion in some people. In my opinion, take this drug when pain is unbearable.

Viral infection might be causing anorexia and other symptoms.

You should just follow advice of your urologist. Do not worry. No need to go to another urologist. You will be fine soon.
