Good evening. Thank you very much for your question. The degree of aggressivity in dealing with an OP of 22 really depends on the diagnosis.....
I would NOT treat anybody aggressively with an opening pressure of 22. Are you implying with your question that somebody is recommending you be treated more aggressively than with your Diamox that's giving you a 22?
The upper limit of normal is stated to be 20. There are a couple of pharmacological choices aside from Diamox which I almost never use anymore due to its rather unclean set of side effects in some people. I particularly like either TOPIRAMATE (Topamax) or Zonegran (Zonisamide). They are much less nasty with their side effects.
Usually, we don't get too terribly excited about CSF pressures unless they are above 28-30 or greater. That might draw my attention for more surgical approaches if clinically necessary. In other words, I wouldn't typically treat NUMBERS so much as clinical symptoms as well as what the neurological exam shows.
Of course, there is a surgical procedure referred to as the placement of a VENTRICULOPERITONEAL SHUNT,,,but again, I think that would be incredibly UNNECESSARY in your case since your pressure is so close to normal that I might even question whether or not the person who measured the pressure actually did it the CORRECT WAY. There are right and wrong ways of measuring CSF opening pressures.
