Question: I am 56 years old, exercise four times weekly, am 5' 10" and weigh 250 lbs. Cholesterol is 141 treated with Crestor (20mg), and BP is maintained with Cozaar (10mg) daily. A1c is 5.6, and I take Metformin 500mg twice daily. Fasting glucose is 98. My brother just passed away at age 60 from XXXXXXX and my AST was 78 and ALT was 50. Are these indications that I might be in early stages of NASH? I do not drink alcohol. What can I do (I'm on a diet to lose weight) to bring the AST and ALT back into normal ranges?

Brief Answer:
Possibly Fatty Liver

Detailed Answer:
Hello,
Thank you for trusting HCM

Your AST and ALT levels in only very mildly increased and could be due to a combination of fat deposition in liver (fatty liver) and the medication Crestor. In XXXXXXX (Nonalcoholic steatohepatitis) there will be inflammation of the liver due to fat deposition. In XXXXXXX AST and ALT levels will be much more increased.
Ultrasound and CT abdomen can confirm the presence of fatty liver.
Fatty liver has the possibility to progress to XXXXXXX You are high risk for XXXXXXX due to positive family history, diabetes, cholesterol, more than ideal BMI.

The good news is that you are on the right track in the management of fatty liver and preventing progression to XXXXXXX
Continue Crestor, Metformin, healthy diet and exercise daily. Avoid alcohol as you already do. Include more vegetables, whole fruits, fiber rich food like oat, XXXXXXX in your diet. Include protein rich food like chicken/turkey breast, soy beans, lentils etc. Reducing weight can greatly help in reversing fatty liver.
Include more healthy fats in diet like avocado, fish including salmon, mackerel.
Follow the above measures and recheck Liver function test in 3 months.
Your current AST, ALT levels are not too concerning.
Following healthy life style and diet will normalize AST, ALT levels in 4-6 weeks.
