Question: Is there any relation between menstruation and the appearence of heavy sudden pigmentation of the face?
Hyperpigmentation under the eyes and on cheeck bones yhis episode happened when she was about 12 y o and she atributed it to an incident when she was menstruation and it was raining hard and got her hair wet apter that she said she felt confused and sick and within hours patches of pigment appeared on her cheeks this was over 20 years ago and the hyper pigmented patches are still there . and as for reducing the impact are there creams wich really work?

We most commonly see this in pregnant women (I am not suggesting that she is pregnant). It is a hormonal effect on the skin often causing darkening of the skin on the face, around the nipples and in the midline on the abdomen. No one really understands the exact mechanism, though it is likely not harmful or an issue except for the cosmetic concerns.

Given the onset at about 12 years of age and the onset of the normal cycle of estrogen and progesterone with the cycle, this seems like a sensible explanation. The explanation is supported by the observation that the onset of melasma is associated with pregnancy or use of oral contraceptives in approximately half of cases. In fact, estrogen and progesterone receptors have been demonstrated in human melanocytes (the cells that produce this pigment in the skin).

So, I would not be too concerned but at least you have a condition that you can google and research further on your own - Melasma. See if this fits and please bring it to the attention of her doctor when you get a chance.
