Suggest Treatment For High Platelet Count

Question: My wife 46 yrs old is having high platelet count of 12 lacs now. 2 months back it was 9 lacs and gradually increasing to this level. jak 2 & crp are negative.

I have gone through your question.
I can understand your concern regarding your platelets count.

Your wife has really very high platelet count. She may have platelet disorder like essential thrombocythemia or may have reactive thrombocytosis. But generally in reactive thrombocytosis platelet count are not this much high. So you have more chance of essential thrombocythemia.

You should go for bone marrow examination once. And also tell me symptoms of your wife. Because for the diagnosis this details are required. Only jak 2 mutation negative does not rule out the essential thrombocythemia. Many a times jak 2 is negative and patients still have essential thrombocythemia. So just send me all symptoms and if possible report of your bone marrow examination.

She is perfectly alright otherwise. She is currently put on ecosprin for last one month. After having this medication, having some momenary pain in the lower ribs/ thigh joint lasting few seconds 6-7 times a day. Have not yet done bone marrow test. May meet our hematologist on coming tuesday. Attaching the reports.

Hi dear,
I have gone through your all reports. All of them suggest you have high platelet count only. No other abnormal findings are there. CRP is also negative so mostly it’s not inflammatory. Again, jak 2 mutation is negative but it does not rule out essentials thrombocythemia. So only option we are left with is the bone marrow examination. It is necessary to diagnose or rule out myeloproliferative disease like essentials thrombocythemia. So consult your haematologist and plan for bone marrow examination. It will clear all doubts. In addition, it will make picture clear.

Hi,
I can understand your concern. Most probably bone marrow examination will clear the picture. Generally it is opd based procedure. It takes half an hour for procedure. It is slightly painful because we have to insert bone marrow needle in to bone to take marrow material. However we can use local anaesthetic drug to decrease the pain. Hope it will clear your all doubts and reach to the diagnosis.
