Suggest Treatment For High Fever In Child

Question: Since 14.12.2014, my son , 4 and half years old(17.5 kg) , was suffering from fever. He was treated by paracetamol for two days in home..his fever was 102 F. He was also having nausea, and abdominal pain. There was no symptom of cough and cold. After two days, as he was not improving, and his fever was once as high as 104 F. We consulted with pediatrician who prescribed zanocin syrup 5ml twice and suggested urine examination and culture. His Routine urine test report came next day and it showed PUS cell 10-15 WBC/hpf, RBC negative, epithelial cell +, casts granular 1-2/LPF proteins-present 2+ (100 mg/dl) and rest was normal value.and the dr. suggested he may need amikacin injection for 5 days. But we thought we should wait for culture report and he was not given amikacin. We waited for 2 days, culture report came negative with no growth. The Dr. suggested that fever was coming from tonsil infection as he was having sore throat in addition to UTI (possible) and prescribed TAXIM-O and azithral instead of amikacin. Even, after that, my son was complaining abdominial pain and fever did not diminish, rather the gap between two doses of paracetamol was shortening. In the meantime, his preliminary blood report showd WBC 22.7 thou/micro L, neutrophil 74, lymphocyts 17, morphology: RBC predominantly normocytic normochromic with mild anisocytosis, WBC-granulocytes show mild toxic change. rest was normal. platelets was adequete. widal test normal. he was reluctant to eat and becoming weak. He took him to hospital on 19.12.14 where he was treated with MONOCEF and amikacin IV. His CRP value was 111. Dr. suggsted that he was having very high infection. His blood culture report was negative with no growth. Within two days, his condition improved, fever diminished and he started taking food. But the doctor suggested to complete the course of antibiotic for 7 days and then he was discharged on 26.12.14.
on, 20.12.14 his urine test as again done and it showed pus cell of 2-3 /hpf, epethelial cell-occasional and bacteria scanty, albumin-nil, rest were normal. Dr. suggested his urine report has become normal and no growth was observed in culture. Additionally his USG report of whole abdomen was normal.
Dr. suggested that his infection type could not be confirmed from culture report as he was already with antibiotic course and possibly he was infected with uti. But it is not confirmed. Throughout this period, he was passing foul smelling gas. however, after two days of IV treatment, his fever and abdominal pain decreased.
I would like to know whether he was suffering form uti or any other kind of infection? As the culture test came negative, we would like to know the possible infection other than uti.


Kindly also suggest the precautions to avoid such a infection again.

Hi....after going through your details, I feel this is UTI for these reasons -

1. Very high total count
2. More than 5 pus cells in urine
3. Pain abdomen and vomiting
4. Poor appetite.

Urine culture negativity doesn't rule out UTI. Culture can be negative for many reasons. By the time you did a blood culture, your son was already on antibiotics. So blood culture negativity is not a XXXXXXX

Precautions -

1. Plenty of oral fluids.
2. Make sure he passes urine atleast 6th hourly once.
3. While bathing him, stretch his penile skin back and wash well.
4. If the prepuce is tight he may need a circumscission.

Thanks a lot Dr. XXXXXXX for your valuable input. As discussed with Dr. , his circumcision is not necessary and earlier he was gently rubbed with lox gel and it was much improved. But, you are right that he was not having bath regularly due to cold and his penile skin was not washed properly. Do we need to take any special care for washing the skin? Should it be dried properly after washing or simply we can wash? Also, we saw some time some whitish reside there sometimes. Thanks for pointing out the fact and we will take care.

Hi...

1. No special care for washing the skin.
2. Simply wash, no need to dry.
3. The whitish substance you saw is smegma.... The slough which is long standing. It will not form when washed regularly.
