Suggest Treatment For High Fever And E-coli Infection
Question: I am a healthy male (sixty five years old), had an e coli infection causing prostititis back in March, ran a high fever for several days, was treated with Macrobid after which I felt better in a few days) and stayed on the medication for thirty days. Early last week, I came down with high fever (102) and chills, frequent and urgent urination, and the urine culture showed the same level of e-coli as in March. Started on Macrobid and began to feel better at end of week. Feeling ok now as my temperature fluctuates between 98.4/98.6 and 99.8/ 100.
I intend to follow up with my physician on everything but three quick questions:
1. In your opinion, is this likely a new e-coli bacteria or could it be the same one as back in March that has now resurfaced? (Intuitively,it seems odd that the same bacteria would resurface and I would have no issues for seven months in between).
2. I have developed a bronchitis and heavy cough with mucus over the weekend, which is unusual for me. Could that be caused by the same e-coli bacteria found in my urine culture.
3. I understand that recurring e-coli infections of the prostate are unusual. Any opinion on what could be making me susceptible to this?

Thanks for query .Based on the facts that you have posted the recurrent UTI that you have could be mostly primarily due to enlarged prostate and needs to be confirmed by relevant tests .
In Enlarged prostate there is obstruction to outflow of urine that results in to significant amount of urine to remain in bladder after passing urine what is called as Post Void Residual Urine (PVR).This residual urine is the source to get repeated episodes of UTI.
I would suggest you to consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis with specific mention of POst Void Residual Urine
3) Serum PSA.
Further treatment will depend upon result of these tests and final diagnosis.
As regards your queries I would like to address the same as detailed below.
1) since the primary cause for UTI that is Post Void Residual urine due to enlarged prostate remains untreated there there is every possibility of organisms to grow again and cause UTI symptoms .
2) The bacteria in urine can enter in to general circulation and cause Bronchitis ,
Doing a sputum culture will help to find out and confirm the same .
3) The cause of recurrent infection is explained in details as above .
