Question: Hello, I received an re-absorvable stent on 12/19 in my RCA for a 90% blockage. I am taking clopidrogel 75 mg and asprirn 81 mg daily. They are in addition to bp , cholesterol and tiglyceride medication that I had already been taken. Since taking the Plavix and asprirn I have felt daily headaches as well as palpitatons and hot flashes on and off. Are these normal side effects for Plavix and Aspirin? Also while I am on the Plavix and aspirin could I keep taking my daily CoQ10?

Brief Answer:
yes its a rare side effect of plavix!

Detailed Answer:

Hi
Firstly thanks for choosing health care magic for your query!

Have read all your details, its a standard procedure post angioplasty to keep the patients on Dual anti platelets for one year.
Flushing ,palpitations and headaches are rare side effects of clopidrogel(plavix).

Palpitations is also found among people who take Clopidogrel, especially for people who are male, 45 plus age , and have started the medicine currently , .A study published in Journal Of XXXXXXX College Of Cardiology states -
" Out of 24,000 people who have side effects while taking Clopidogrel from FDA. Among them, 163 have Palpitations and 390 have headache and nausea".

Though such side effects are not encountered with aspirin.

You could ask you cardiologist to shift you to Prasugrel or Ticagrelor(Brilinta) along with aspirin.

As all other medicines mentioned by you , you were taking them before angioplasty , so most likely culprit in your case is clopidrogel.Ask you cardiologist to shift you to prasugrel or brilinta.

Prasugrel is reported to cause headaches in about 6% cases while ticagrelor causes headaches in less then 1% cases, which although is more then clopidrogel; but in medical science the golden rule is that there is no rule ,so you can ask a cardiologist to give a try.

Similarly a study of 7,087 people with Brilinta ,reported palpittions among 43.

So my suggestion is don't worry just ask your cardiologist to switch over to another drug.
Yes CoQ 10 is highly cardioprotective and can be taken daily as a supplement.

In case you have any other query i would be happy to help!

Thank you,

thank you for your response and will inquire about the 2 alternatives you mention. I also forgot to mention that I have now developed a constant ringing in my ears. This has happened as well when I got on the plavix and asprin.

Also this may be a bit premature but when the time comes to come off from the I have read there is in increase of having a stroke or heart in the first 90 days of stopping the plavix. Does this mean that I am now stuck taking the plavix or other blood thinner for life? I have read that the potential of a deadly heart attack or stroke is increase within this time period and I am concerned about this. Will I be stuck on plavix or other for life?

thank you

Brief Answer:
Tinnitus is a side effect of asprin!

Detailed Answer:
Hi again,
Ringing in the ear is called tinnitus,Tinnitus is the perception of noise or ringing in the ears. Tinnitus isn't a condition itself — it's a symptom of an underlying condition, such as age-related hearing loss, ear injury or a circulatory system disorder. However aspirin has Tinnitus as a very common side effect.Aspirin, can cause or worsen tinnitus. Tinnitus is often associated with hearing loss and Aspirin potential effect on hearing ,Hence i would suggest you to go for pure tone audiometry,a test for hearing.

No its not so, you can shift from plavix to other blood thinners as per instruction of your cardiologist , stroke and all are very rare case scenario(bookish thing ,very very rare in real life), but plavix and asprin will be continued for the whole life ,stopping them will cause restenosis of stent , basically they are very safe drugs and complications are very very rare. You can shift from plavix to clopigrel after getting a loading dose of 300mg followed by 75mg twice daily by your cardiologist .However cases of stroke in long term use of prasugrel are much more then clopigrel;which is a much safe option!

In case you have more query ,feel free to ask!

Thanks

Thank you Dr. Saxena.

I will request the audiometry test as suggested.

So I am currently on coplidogrel 75 mg once daily and aspirin. My concern however is not switching blood thinners. It is more to when the time comes to stop the coplidogrel. At some point in perhaps one year or two I have been told I will stop the coplidogrel and continue on Aspirin alone for life. At that point it is my understanding that platlets at this point are more aggresive or from more aggresively than normal due to the absence of coplidogrel. This is called the rebound effect. So what I have read is that during this time period within the first 90 days of stopping this or any other blood thinner there is a known increase of fatal stroke or Myocardial Infarction due to this known "rebound effect"

So knowing this my question is when the time comes to stop taking the colidogrel is it safe to discontinue it or am I stuck taking it for life due to this "rebound effect" that seems to happen?

thank you

Brief Answer:
They are all very rare things!

Detailed Answer:
Hi Again,
Frankly speaking a doctor do whats written in a standard text books, which says if a bio-absorbabale stent is implanted ,patient is given dual anti platelet therapy for 1 year and after that single anti platelet is used life long.

There is no specific mention of rebound effect in any of the standard text books like -BRAUNWARDS CARDIOLOGY(Bible of a cardiologist).

So ,Don't google too much ,they are rarest of rare things. Don't worry follow suggestion of your cardiologist ,you will be fine!

Its a part of standard protocol to shift a patient from dual to single anti platelet in a year!
