Suggest Treatment For High Blood Sugar Levels While On Diabetic Medication

Question: Dear Doctor,

I am having type II diabetes from the last 3-4 years and I am Taking Amyral 3 mg before breakfast and then Metformin 850 at lunch and dinner time. Also I am using Throxine Tablet 100mg also before breakfast. My questions are as follows:

1. Being a specialist do you approve these medicine or there are some better medicine.

2. My blood sugar level in the morning before breakfast is always little high (between 140-50) but in the afternoon at random checking it is usually. My Hb1AC on all occasions came between 6.0 to 7.0. My question is this that why sugar level in the morning is always high and can you suggest something to solve this problem.

3. Can i change the timing of Amryal and Thyroxin tablet because psychologically I dont feel good to take first thing in the morning before eating anything.

4. Any other useful advice to fight againt diabeties.

Amaryl and Metformin are fair choices but there is no such thing as better or worse. Everything is individualized to the patient. Ask your doctor if you are a candidate for other diabetes medications such as Sitagliptin or Voglibose or Empagliflozin

Morning sugar elevations are a common problem in diabetes. A higher dose of metformin could be helpful. Consult your doctor to see if this can be done.

Amaryl needs to be taken before breakfast while Thyroxin must be taken on an empty stomach 60 minutes before breakfast with water only.

4. Any other useful advice to fight against diabetes.

If you are overweight or obese you need to lose 5-7% of your body weight , and eat healthy and increase your physical activity

thanks for your answer and it is good... I have also little high cholestrol (between 230 - 260) and also tryglyceride are little more than normal.... Can you suggest me any tablet which help to reduce pls//

When I see someone like you in my practice, I typically order the following blood tests in addition to a detailed physical examination:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
HbA1c (Glycosylated Hemoglobin = your last 3 months' glucose average). Also known by other names such as GlycoHemoglobin or Glycated Hemoglobin or A1c
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH (checks your thyroid)
12 hour Fasting Lipid profile
Urine albumin to Creatinine Ratio (early sign of diabetes affecting the kidney)
25 hydroxy Vitamin D levels (ideal range 40 to 60 ng/ml = 100 to 150 nmol/liter)

Correct diagnosis and treatment requires the opportunity to examine the patient so you must see an endocrinologist in-person.

For example, an abnormal thyroid can lead to high cholesterol problems. The cholesterol values that you have mentioned do no have the LDL and HDL which are also important targets to treat. When you see the endocrinologist, (s)he will likely prescribe the appropriate type of cholesterol pill based on the above comprehensive assessment
