Question: hi doctor i am diabetic- type 2 since 5 years in spite of regular medication, physical activities & following all diet guidelines my sugar levels continuously remain high ( Fasting 250 + / PP 350 + )

how can i control ?

Noted your concern.

You are already on almost 5 different types of medicines.
Tab Gemer PL 2 contains Glimepiride 2mg + Metformin 500mg + Pioglitazone 7.5mg
Tab Istamet contains Sitagliptin 50mg + Metformin 500mg.
Tab Forxiga contains Dapagliflozin 5mg.

Despite of that your blood sugar levels are not under control.

In such case I may suggest to use of insulin particularly night dose of long acting insulin like Insulin Glargine along with current oral medicines
With regular check of blood sugar level.

Along with this one need to consider the possibility of LADA (Latent Autoimmune Diabetes of Adult).

All this need to be discussed with your treating diabetologist.

above medication just started before 2 days earlier i was taking combiglyze 1 tab daily and i was out of medication before 7 days of last test result i mentioned as above

In such a case I may suggest to continue current medications as advised by your doctor.
Be compliant with medications, diabetic diet & exercise.
Recheck blood sugar level Fasting & Post-prandial after 2 to 3 weeks.

On the day of blood sugar testing one should maintain the daily routine including meal timings, timings of medicine intake.
Do not make any changes in medications or discontinue any medications without consulting your treating doctor.
After doing blood sugar tests (whatever the test result) continue your medications as advised till you consult your doctor.

There is likely possibility of reduction in number of medicines in future depending upon blood sugar levels.
