Suggest Treatment For High Serum Creatinine Levels

Question: Hello i am 37 year old male with history of hypertension and hypothyroidism for last 15 years..I started medications for bp and thyroid 10 years ago..I tried diffrent medicines like telmisartan amlodipine and hctz..My bp used to remain 150/90..Now i am on olmesartan 40mg and levothyroxine 75mcg..The concern is my high creatinine levels.My creatinine levels are 1.5mg/dl.Urea and uric acid are 17 and 4 respectively..Do I have ckd? I suffered from UTI several times with blood in urine in 2004..USG AND IVP are normal..I took statins and fenofibrates also for high triglycerides..I have heard fenofibrates cause elevted creatinine..What is the next step i should do.Should i completely avoid non veg food..Why my urea and uric acid are always normal and creatinine is high..I am scared..Plese give best possible advise.Blood sugar is normal..

There is nothing to be scared of.

Serum creatinine 1.5 is slightly high.

We need to see an ultrasound KUB report , urine routine report and hemoglobin report to comment that is it a CKD or not. Overall as per your history it is likely to be CKD as the duration of problem is more than 3 months.

Causes contributing to increase creatinine , other than CKD, can be-

Use of protein supplement powders, use of pain killers, use of statin and fibrate combinations ( you can stop them), even olmesartan can raise creatinine levels.

now what we need to do,
diet- vegeterian, only occasional non veg and in less amount,
no tobacco at all,
no aerated drinks or juices.
keep bp less than 130/80 with help of medicines
repeat serum creatinine every month to see trend if rising or improving

Overall, there is nothing to be scared of, be in regular follow up with your doctor.

My USG KUB IS NORMAL AND IVP ALSO..URINE CULTURE SHOWED INFECTION FOR WHICH I TOOK ANTIBIOTICS..HEMOGLOBIN IS 14.5..I TAKE CONTROLLED DIET..SHOULD I UNDERGO CREATININE CLEARANCE TEST?

Hi,

You should note the size of kidneys in centimeters., just saying they are normal is not enough. also we need to see any difference in the size of kidneys any cortical scars and post void residue .

There is no need of creatinine clearence. Repeat serum creatinine every month to see trend if rising or improving.

Also you should be in regular followup of the doctor.

As my current bp remains 125/85 with olmesartan 40mg should i continue with the drug? Yes kidney size was mentioned in USG report and both were normal and there was not a mention of any cortical scars or cysts in kidneys..Should i start yoga? Also please tell me can i lift weights as i need to loose 10kg of weight..

please continue the medicine, BP is good, yes you can start yoga.

To loose weight, lifting weights is not a good exercise.

for that you need aerobic exercise, walking, running, cycling, elliptical, badminton like that.

This is my last question to you sir..
My last reading of creatinine ad on 7/8/15 was 1.10 urea 17 and uric acid 5...I will check creatinine again in a week..I want to ask about XXXXXXX medicine vrikdoshar kwath..There are very good reviews about it on forums and youtube..Do you recommend taking it..Also please tell me about role of baking soda in CKD..There is a study about taking 600mg twice daily in water before meals..It seems to neutralise all toxins..Please give your advise on these two things..

sodium bicarbonate ( Baking soda) in a pure form is available as tablets can be taken 500 mg twice daily in kidney disease ( or as per your treating doctor) . But you should watch your blood pressure , it should not rise.

I have no knowledge of ayurvedic medicines . you should consult an ayurvedic doctor for the same
