Suggest Treatment For High BP

Question: for 18 years my blood pressure hase been 120-130 over 75 to 84, I decided to take some weight off from 226 lbs to 210 lbs in 16 weeks. About half way through my blood pressure went up significantly over a few days and I also felt somewhat dizzy. I take felodipine ER 5mg and valsartan HCT 160/1. I reduced my calories during this diet to 1200 - 1500 per day. My colesterol levels are with in the normal range and have been for years. I exercise 30 minutes hard everyday. I an 67 years old. 6'2" 210 pounds.
What has happened to me to increase the blood pressure during weight loss?? than you in advance

I am so sorry to hear about this increased blood pressure that you have mentioned. I will like to know what your blood pressure is at the moment and whether this has been constant or not. Of course we would have expected your blood pressure to drop with a drop in your body weight but this is clearly not to be the case.

My thinking is that you might have applied very strict and harsh measures in an attempt to loss weight that has created a residual and constant level of stress in you.. This could offset the advantages that decrease body weight would have brought in. The good news however is that as time goes on and these measures become more of a habit that active efforts, the stress would give way and you would observe a steady decrease to more desirable and steady blood pressure numbers.

My blood pressure on a typical day, is as such
8am 150 over 95
10 am 140 over 92
1 pm 135 over 90
3 pm after my half hour exercise 118 over 85
5 pm 120 over 88
8 pm 140 90

15 weeks ago before the weight loss which was not very difficult as I had food in small amounts every 2 hours of the day, therefore never got hungry.
what I noticed is that now my bottom number seems to want to stay higher then before the diet started 15 weeks ago.
I have no sugar diabetes however sometimes I feel somewhat wonky/light headed, also sometimes hissing in my ears.
I thought maybe i was having low blood sugar problems from the diet, yet my glucose meter shows no such problem.

Hi and thanks for this follow up.

I have reviewed the numbers. They seem highest in the early hours of the morning and taper off during the day. What medications do you take and at what time do you take it?

My thoughts are that either you sleep poorly at night and it accounts for this high early morning figures or that the medication you take is unable to do a good job overnight to maintain the blood pressure normal. Having a lower blood pressure after exercise in the afternoon compare to the early morning readings is not very typical.

Let me hear from you and together we would revisit these findings and make the necessary adjustments.

I sleep well 7 to 8 hours , rarely tired during the day.
For about 15 years I took Valsartan 160/12.5 mg and adalat xl 160, for that period of time I had stable blood pressure. I moved to another city 3 years ago and when IO went to refile my prescription my new doctor immediately switched me to felodipineER 5mg away from adalate 160 xl, my blood pressure did stay the same(old normal) however till 15 weeks ago when it became erratic half ways to my weight lose program.
When I asked new doctor why the sudden change in pressure he said it was just old age and my blood was normal. I asked if maybe I should be back on my old faithful adalate160 xl. He seemed not happy that I asked that question for some reason.
I have never smoked or consumed alcohol other then a wine at a weekend dinner and only in small amounts. Right now my BP is 132 over 88 and starting to climb for the night.

Hi and thanks for this follow up precision.

The current blood pressure is great, 132/88mmgHg, What I seem not to understand is why it would rise by night. Felodipine ER maintains stable levels over 24hours and should be able to work effectively and not fade as the day goes on.

I may think it is a coincidence at this time with poorly controlled blood pressure and weight loss efforts and not a direct cause -effect relationship. I will suggest that you get more measurements over 3days using the same time frame and present to me. If the values are consistently erratic, it may be time that your prescription be adjusted to get this blood pressure well controlled. 15weeks is a long time and waiting would no longer the best idea at this time.

I would not change a drug that works well and more so on the first meeting. I have no idea why he did so without proper communication with you.
