Suggest Treatment For High BP

Question: Hello Doc.,
My name is XXXX. I am suffering for High BP and on medication for the last 2 years on Telma-40 (once daily) & Metpure-XL12.5 (once daily). The situation was under control and it was more or less near abt 130/90. Now for the past 2-3 weeks, when I had taken BP, it was again on incresing trend with BP near abt 150/100. I am a smoker and taking 5-6 cigerrates daily.
Can u suggest what medicine I have to change?

According to the current guidelines for Hypertension, you would need to modify your treatment.
Your Blood Pressure of 150/100 mmHg is high as per your age. I would help you in modifying your treatment.

I have seen your reports, and appreciate them. But I would like to know a few more details about you:

1.What is your resting Pulse rate? You can measure it for 1 minute and then tell me.
2.Do you consume alcohol?
3.Do you have any headache or palpitations?
4.Do you have any vision problem?

This is my best opinion for you based on available details. If you have any further query please ask me and revert with the answers of the questions preferably in point wise manner.

Dear Sir,
I am working in office and have normal routine office working atmosphere mainly on computers. There is very less physical activity involved.
Point wise answer for your queries is follows:
1) The resting & normal pulse rate is more or less same and it is in the range of 72-80.
2) I am taking Alcohol socially, may be once-twice in a year and that to also restricted in consumption may be 1-2 pegs.
3) No, I am not suffering from any headache, but yes, I feel palpitations some times occasionally may be once in a 2-3 months or some times even more than 4-5 months for 5-10 secs.
4) I am using Specs and regularly doing check up with eye doctor. From the last one year I am using the same nos, although I got it checked around 2 months back and same number was advised by eye doctor.
Addition to this, I want to add that I am doing mix up exercise: walking & runinng . And, I am doing near abt 3.5 km in 40-42 mins. But, the same is being done 1-2 times in a week due to my hectic schedule.

Dear Friend.

After reviewing your reports and query, i advise you following:
1. To stop smoking
2. To take Low salt diet
3. To Start on Tablet Telma-H 80/12.5 once a day.
4. You can stop Metpure XL.

After 10 days of starting on this medicine, get your Blood Pressure checked by Manual BP Machine and review with me.
Decision of adding Metpure XL would be taken based on your response to Telma-H.

Current guidelines state that a drug like Hydrochlorothiazide should always be added at your agent as first choice of Anti Hypertensive.

Dear Doctor,
a) Thanks for u'r prompt reply. I am already on low sodium salt available in market. I will start the medicine check my BP on Manual machine. Will there be any side effect if I am taking this medicine on a long course of time.

b) I want to ask one more query out of my curiosity that what is the guideline / criteria for prescribing Telma-H & Telma-AM. i.e when do you go for Telma-H and when do you go for Telma-AM.

The reason for asking this one is as in office I find some persons are taking Telma-H & some on Telma-AM. As far as I know, Telma-H is extracting the excess salt from body and Telma-AM is working on elasticity of the veins.
Do correct me, if I am wrong.

There will be no side effects on changing the medicines.

Using Telma-H and Telma-AM, depends on the level of hypertension, heart rate and patient's age.

In your situation Telma-H is the best drug for you. Hydrocholrothiazide has been shown to reducing long term morbidity and mortality in patients with hypertension, so is essentially first line drug these days.

If you want to read you can read joint national committee 8 guidelines for hypertension, that is followed world wide, and is well accepted.

I would like to review you again after 10 days with your blood pressure and pulse rate as i would decide that further addition of Metpure XL is required or not.
