Brief Answer:
Conservative treatment should be tried first.

Detailed Answer:
Hi, I have gone through your question and understand your concerns.

From your prescription, the diagnosis is Herniated disk disease at L4-L5 level.

Treatment for that condition, especially at young ages, consists on restriction of work/weight lift, NSAID drugs, physical therapy and selected exercises.

When these measures fail to improve symptoms, decompressive surgery for herniated disc disease is needed.

About kneecap condition, if the specialist Doctor ( in this case Orthopedic surgeon) recommends replacement, it should be done.

These possible treatment ways should be discussed with your wife's primary care Doctor.

Hope this answers your question. If you have additional questions or follow up questions then please do not hesitate in writing to us. I will be happy to answer your questions.

Brief Answer:
different stages.... .

Detailed Answer:
Hi again.
Disc protrusion is the first stage of disc herniation, where the disc is stiil in one piece but compressed.

Disc herniation occurs when this compression goes further and the disc is broken, leaving the soft part of it outside in the spinal canal or nerve roots, pinching them.

Hope this helps. Best regards.
