Brief Answer:
usually not a big pronlem

Detailed Answer:
Hi
It's often alarming to note blood in semen but almost in all patients it is self resolving. There could be causes like minor trauma or infection.
You can get a few tests done like ultrasound of scrotum, a semen analysis and semen culture, and a transrectal ultrasound.
In the meantime you can use a scrotal support, use mild antiinflammatory drugs. If thetevua any evidence of infection you can use a broad spectrum antibiotic like ciprofloxacin.
As I said, please do not worry. It is almost always never serious.
