Suggest Treatment For Heavy Growth Of Enterococcus Faecalis

Question: Hi Doctor, thank you for following up with my case.

After seeing a local Urologist, he was not convinced that I had a yeast infection, so he told me to go back to my family Doctor to get a culture done and check for diabetes just in case.

Luckily, my A1C came back within normal range, as did my C3 and C4, as I asked them to check up on my immunity indicators while they were taking blood.

Unfortunately, the skin culture came back with heavy growth of Enterococcus Faceilis (sp?). My urine and blood are clear, so it seems to be only on the skin of my penis and scrotum.

I have just completed 10 days 4x of Ampicillin, so my family doctor just took another skin culture on Wednesday, so I have yet to hear back.

I still feel symptomatic, itchy, a chaffing feeling on my scrotum when I wear jockey underwear is apparent as well. My doctor told me to think positive and hold off on any medication in order to see how this latest skin culture comes out.

Any thoughts are welcome, as I am quite worried.

Brief Answer:
Urine culture is mandatory.

Detailed Answer:
Hello
Thanks for your fresh query,After going through the thread of our previous discussion it seems that you have not done Urine culture and swab test which I have suggested in my reply to your query earlier.

You seem to have have acquired Streptococcal Faecalis infection of the Urinary Tract .

Your family Dr has rightly started giving you Ampicillin .A combination of Ampicillin and Gentamycin is more effective in Sreptococcal Feacalis Infection .
Please discuss this with your family physician add Inj Gentamycin twice daily in addition to the medicines that you are taking..

Do not worry any disease for that matter is not permanent and one has to face the situation boldly .Please remember that there is always a light at the end of darkness in tunnel.

Hope I have answered your query ,please feel free to ask if you have more questions
I shall be happy to help you
Thanks and Regards.

Thank you Doctor, but my family doctor only gave me a 10 day course of Ampicillin, which I finished three days ago. He wrote another 10 days of Ampilcillin but said to only take it if I felt the infection was coming back?

He did do a swab test on the skin of my penis, which showed the growth of Enterococcus Faceilis on the skin. Each of my urine tests came up with no infection, yet just last Wednesday he is sending out for Urine Culture, which I asked for.

So I am trying to do everything you suggested. I am feeling a little better today, as the itching has calmed down, and I have no pain while or after urinating. My doctor believes I had a bad reaction to the fungal creams so I developed almost a burning inflammation that went away after discontinuing the creams.

At this juncture it has not shown up in my Urinary Tract, besides some discomfort in my Urethra. Would this be considered my Urinary Tract eventhough it doesn't present itself in my urine?

Waiting on my both urine and second skin swab cultures from Wednesday.

I will relay your medication recommendation on to my doctor, but he currently doesn't have me on any medication? He is hoping the Ampicillin alone has done the job?

Brief Answer:
It is fine not to take medications if you have no symptoms.

Detailed Answer:
Hello
Thanks for follow up.
As you have already finished a course of Ampicillin for 10 days and you do not have symptoms .

Please note that Streptococcal Faecalis Infection has tendency to recur again and takes long time to get cured completely hence one needs to take medicines for 3 -4 weeks.

Wait for reports to come ,in case the infection still persiss take Ampicilin and Gentamycin for another 5 days followed by Ampcillin for another 10 days.

Ensure to wash your genital area with warm water twice daily.

Please do write to me once you get the reports.

Thanks and Regards.

Hi Doctor, talked with my family doctor yesterday.

My Urine Culture came back negative, no growth.

Still waiting on the skin swab, which is where it tested positive last time. I will let you know as soon as I hear back on it.

Just thought to keep you updated.

Thank you.

Hello Doctor,

My Skin swab culture came back this morning, which stated I have "Low Growth" of Enterococuus Faceilis. The sample was taken from the skin just under the head of my penis.

And again, the urine culture came back negative.

Both of these lab cultures were taken on March 18, 2015.

As my family doctor is out sick currently, I have set up an appointment for tomorrow morning with a Urologist that I have been to in the past. My appointment is scheduled for tomorrow morning at 9AM.

Your advice is welcome?

Brief Answer:
You still have mild Streptococcal Faecalis infection.

Detailed Answer:
Hello
Thanks for follow up.

Your recent swab report suggests that infection is not fully controlled Hence you need to take Ampicillin for 10 more days .

You can discuss with your Urologist about taking Gentamycin along with Amicillin whom you are meeting ton Monday.

Thanks and Regards.

Hi again Doctor, just came back from my appointment with my Urologist.

He said that everything looked perfectly normal and to stop having skin cultures done by my family doctor. He said to leave everything alone and to not take any more medications, period. He is afraid for me to take any more medication unless he sees it as absolutely necessary. I told him that I still felt symptomatic, but he said that the last time he saw me regarding this that I had bad chemical burns from the fungal creams and that the skin was still healing. So that is why we should just let it all alone now to let it completely heal.

I am hoping for the best.

Thank you Doctor. XXXX
