Question: Since the past 11 months I have been having anxious all the time. Might be anxiety disorder but I never experienced something like this before 11 months. I had been leading a content life with no strees before 6 months and never knew why people had anxiety or what is anxiety at all.
It all started 11 months ago when I was in my bed late night and I felt a very very very little uneasiness on my left lower breast side (just Below the breast actually). I started thinking what if its related to heart...what am I gonna do if its some heart problem...and then I started to think deeply and couldn't sleep and started panicking and precipitating a little. Got up from bed and then splashed water on my face and head but still couldn't sleep and it was already morning before I could sleep. Now since that day I have been constantly thinking abt my health and all that. Its health anxiety I know and I cant think of anything else except health and thats it..its giving me a hell of a lot of tension. I went for blood tests and everything was normal but still I couldnt believe the reports and have been a worried person ever since. I am in Good health though and the only exercise I do is pushups and evening walk of 3.5 kilometres daily. I am not obese and my weight is just 60/61 kilos (height 5.7) . However because of unreal fear I stopped doing pushups at all for the past 4 months...(like I thought what if I fainted doing pushups) I am giving you a physical description of mine since you are unable to see me in person.
Going further; I wd say that I have become mentally weak as even something like a mild earthquakes wd freak me out and and my heart wd start beating faster for a few minutes. I always wd have bad thoughts running thru my mind. Sometimes I wd feel like my brain is going weak.
Nowdays I am observing that my body shakes a little with every heart beat and I can notice it when I am too much conscious of it . This is more apparent when I place my elbows on my things while sitting. But this shaking is not noticeable above my neck. And also the whole shaking is very little to mention about but since as of now I have gone a bit timid I keep noticing it and feeling it all the while when I try being attentive about it. However my heart beat isn't that thumping or beating that hard. It even beats within the normal range ..sometimes 65 sometimes 72 sometimes 75 or 85. Only sometimes when I am too freaked out like if theres an earthquake or even the sense of it ; that my heart beats harder which I could feel with my fingers placed against the neck artery. Apart from that I am a normal being with no unhealthy habits. I never ever smoked or took a drink. And I eat healthy too. Day and night I wd keep thinking abt my heart and I wd get worried abt myself if I hear about some ones' health getting bad. Alsdo at times I wd feel as if theres a slight tremor within my body and I wd feel it in my hands and limbs. But this thing is not noticeable within the outside. I can only feel it sometimes within my body and it wd freak me out and I wd start thinking I am going mad or losing my mind or that my nervous system is wrecked or that what if I am having some nerve disorder. I don't know what to do and I seek your counselling as I am just being unable to get rid of all this and all this is making me weak. I forgot to mention one more thing. Now since a month I am observing that if I bent my knees/ legs or hold them to some awkward positions the legs wd tremble a bit. While this doesnt happen when I am taking a brisk walk or stand or like. It happens for example when I try to hold my legs upward while balancing myself on the bad by keeping my hands behind my back on the bed. Also it happens when I try to hold a leg in the air either by straightening one leg forward at 90 degree angle or if I try to bent my leg in the knees and turn that leg against the sides. It also happens in my lower arms if I try to lift a lid of our household water talk upward from the inside. Its a lid like a door with a hinge on one side of it to be opened from below the tank so that one side of the door is kept tilted and when I push it upward from below using one hand I wd get a little shake on the forehand as if I am balancing some weight but the weight doesn't feel heavy to me in the true sense. Its only normal-heavy but I keep getting the little shakes on the forehand while I do it. And when I think abt it all the time I feel my brain getting a little weak. Some years back I also did workouts with dumbells and barbells with mild weight and I naver got too tired. My body shape is very slim and I never get bulky or muscular or flabby even though I eat well. Please help me as I am tensed 24 hrs a day. Is there any medication to relieve stress like this without having any withdrawal effects? What I mean is if there are any meds available which aren't habit forming and which relieves you off anxiety without giving you any side effects or without harming the vital organs of the body.

I have read the query and can understand your concern. You have mentioned the detailed description of your problem and as you are also aware of this, these symptoms suggest health anxiety only. Health anxiety or hypochondriasis is anxiety occurring due to fear of some serious health concern. The anxiety is related to Obsessive compulsive spectrum disorders. The individuals remain aware of the unrealness of their unwanted intrusive thoughts still they remain distressed.

Same thing is occurring in your case, you accept that your thoughts are not real, even tests prove this but still your persistent, intrusive fear is not going. The other symptoms you have mentioned are also the part of the health anxiety and anxiety spectrum disorders.

Now just to reassure you, you are a 38 years old healthy male with average height and good weight.

If we consider weight as per your height and age then that should fall in range of about 68-70 kg. But your weight is 61 kg and that is good. This also weakens the possibility of any cardiovascular illness in your case.

Your BMI is about 20.8 which is absolutely normal and this also puts you in safe category of cardiovascular illness.

Your all tests came normal, you walk daily for about 3.5 km which also don't favour the risk of cardiovascular illness.

So try to reassure yourself but these datas. You are not at all suffering from CVS illness so no need to worry. All this is anxiety only and will resolve with time.

So please visit a psychiatrist for treatment. Medicines like SSRIs are totally safe medicines for such type of anxiety. These medicines don't cause dependence and can be tapered off easily once the remission is achieved. Escitalopram, Citalopram, Sertraline etc are medicines that can help to reduce these unwanted thoughts. For severe exacerbations other medicines like low dose benzodiazepines can be used. What abt the mental weakness (please read my whole problem once again) that I wrote about and what about the little shaking of my body along with the heart beat which I can see physically (and sometime I even feel little shaking in my legs and hands and inner torso which can be felt only from inside). Also, abt the little tremors I get when I try to hold things and hold my body into some positions I described. Please read below for reference'

I have read the details already. The feeling that brain is getting weak is occurring due to anxiety only. The earth quake like feeling of body, trembling or shaking of body, legs trembling in certain position etc symptoms are part of autonomic excitation seen in anxiety. In anxiety sympathetic nervous system gets stimulated and thus cause similar symptoms. This is not occurring duet to some nervous system problem or due to some neurological lesion.

In anxiety brain couldn't focus in a thing and thus appears to be getting weak. These symptoms can't be explained by any neurological disorder so there is no need to worry. Just remain hopeful and take proper treatment of your anxiety. With time your symptoms will improve.

Ok thanks for your response Dr. Sahab. But I am still confused about one thing. My whole body shakes (hands and back) very mildly with each heart beat and this is noticeable to me when I sit silent. Tell me if this is normal as I am thin and don't have a muscular body. I am just not too thin but quite slim to my age.
And I wd like to know why I get so frightened by things like an earthquake or similar things or situation like two persons fighting physically with each other. This wasn't so like a year before. Situations like this freaks me out and my heart beats fast for a few minutes and this frightens me even more. I get so tensed that I can't even describe.
A friend of mine who experienced similar symptoms like mine some few months back suggested I take "TRYPTOMER 10" as he thinks this med got him cured under 4 months. So I wd like to know under which category of drug this "tryptomer 10" falls...is this a good medicine and is it habit forming? I know I wd sound a complete fool but still I wd ask "is there any medicine that wd help me get rid of "sudden fear" which comes to me all of a sudden out of nowhere even if I am not feeling already anxious. "Sudden fear" such as I described above like an earthquake or for example a noisy scene outside my house. Sometimes even when I am in my bed for a nap during the noon I wd suddenly open my eyes and sense some formidable fear for no obvious reason and I wd get freaked out and get up from bed and then walk here and there just to get relieved or ease out; my heart wd pound hard for a few minutes and wd beat fast. For this very reason I have completely stopped taking nap (or a 1 or 2 hour sleep) during day/noon for the last 11 months. Earlier I wd regularly relish my afternoon naps or sleep. I just don't know what to do.

You wouldn't know how I am living thru each day with fear all the time.
As for the Meds; I want a medicine which wdn't interfere with my heart's functioning as its all abt the heart that I am constantly thinking all day....just can't help it....

Secondly when I am having the anxiety I have a dry mouth..is this normal?

Also, I feel strange sensations in my chest ...don't know if its inner or outer....the feeling is like of extreme fear in the heart area (upper area). I dont know if this is triggering the anxiety or anxiety itself is causing it. However with God's grace never felt any pain in that area but its something of a different kind of feeling (makes me think about heart attacks and heart failures and cardiac arrests...I read abt such a lot on the internet and while thinking about it more and more I get freaked out more and more). On a positive note I don't get that much tired if I walk a kilometre briskly. Tell me please I if am having normal symptoms or not.
What medicines will be good for me?

Question 1) You had said "Sertraline is a safe SSRI drug." But under what or which name can I get that in XXXXXXX I need the low powered dose though..

Question 2) Is this drug TRYPTOMER 10 is dafe for heart too. A friend of mine recommended that.

Question 3) I just had 102.8 F fever today. I was fine till morning and then when I woke up I found myself sneezing and having a runny nose by after the fever came and it was 102.8 F when I checked at noon. I was so freaked out that I took 1 tablet of Sinarest LP hoping to lower the fever. The fever lowered to 101.5 f first and then its 100.00 right now. I feel so desperate because my heart rate which was just fine till morning (Under 80)when I woke up ; it went up to 125 or 120 in the afternoon and its been the same now so I am really having worries. I never had a heart rate that went 95+ except when I used to do push ups (it wd go to 130 bpm). Can fever increase the heart rate?

Question 4) Now can I take etizola beta 0.25 with the tablet sinarest? Can it coz any trouble or interaction. I want to take etizola beta 0.25 just to calm my heart rate as it was the first drug prescribed to me when I first had anxiety 1 year ago. I never took more than 7 tablets of it in 9 months

1) Sertraline is available with Serta, Sertacad, Zosert etc brands. These are good brands and you can choose any. Starting doses are 25-50 mg per day.

2) Tryptomer at 10 mg is safer but in high doses the drug might cause cardiac side effects. The drug can increase BP also.

3) With increase in body temperature increase in heart rate is expected. There is normal direct relationship between body temperature and heart rate. Approximately one degree rise in temperature causes about 10 bpm rise in heart rate. So just remain relaxed, the increase in heart rate is absolutely normal, no need tow try.

4) Yes you can take, no need to worry. The drug will reduce the heart rate and will improve the anxiety also.

1) My normal body temperature as I have recorded during the past few months is 97.8 or 96.8 so it shoot past 102.8 this afternoon and right now its 100.00 so is this fever too high for me? Should I take antibiotics (just asking your view). Have I deviated 5 Degree F from my normal range?

2) My anxious mind tells me usually infection within body causes such high fever. But since 1 tablet of sinarest LP lowered the fever by 2 degrees..do you think its a good indication that I wd recover soon?

3) Should I take 1 etizola beta 0.25 now to lower the heart rate or wait for my body to naturally lower my heart rate when fever lowers? Since etizola wd try to lower the heart rate which is not natural due to the fact as you have asserted that fever increases heart rate.

4) I just checked the heart rate and its 106 bpm..do you think it wd automatically go down to a good rate once the fever is down?

5) Are these questions generally asked by anxiety patients ? Have you had patients asking such questions like I did today to you? Now you see....how anxious I am always

Please don't rely on digital thermometer. If you have doubts get your fever checked with a good doctor. Increase in temperature results in increase in heart beat. The temperature of 100 degrees is not very high so no need to worry.

Yes you will definitely recover from fever soon.

You can take Etizola Beta on SOS basis for heart rate. The drug would help to normalise your heart rate.

106 bpm heart rate is not very high and is not alarming. No need to worry for that. Once your temperature comes normal then the rate would come down.

I just checked the fever its 100.3 and I didn't take the sinarest yet for the day. I last had it at 11:00 PM night. Does it mean I am recovering? Also I just checked my blood pressure its 110/80 on the left hand side and 110/82 on the right hand side. My blood pressure for the last couple of months was 110/70 . Can fever increase blood pressure? is this a proven fact?

Yes this means that you are recovering. Please don't remain anxious due to fever. Fever is a protective phenomenon for body and having fever doesn't mean that there is something serious. Any physiological phenomenon cause variations in BP and your BP is normal range. There is no significant difference between 110/80, 110/70 or 110/82.

Yes fever is known to increase BP.
