Question: My husband has a diagnosed hematoma on his foot, from ankle bone to toes. It was originally black, and after going to E.R. and being diagnosed on Sunday, then follow up with pcp on Tues. it has now turned bright red and is very swollen and extremely painful. He has been doing the RICE treatment, but the change in color and pain level is considerable. Is this considered an emergency, or is this how hematomas behave. He is on Eliquis, blood thinner. He is 75, and able bodied otherwise. thank you.

Were you told why did he develop hematoma suddenly? Was it injury or because of blood thinners?
What is the size of it? Was surgery not an option to remove hematoma?
Hematomas are bright red in color and extremely painful when large in size and they press on the nerves and bones. So it is how the large ones behave.

Please let me know details.

He was on a 4 day fishing trip, and when he came back it was swollen, then our 195 St. XXXXXXX stepped on his foot and within hours it turned black, now bright red. It is about 4" across the upper right of his foot and 4-5" long from the toes to the ankle. Surgery was not mentioned by either E.R. dr or pcp. He and I think it was the combination of doing such physically stressful activity on an ocean fishing trip for 4 days, then the dog stepping on it. I would imagine Eliquis plays a part in it, as do the doctors. I very much appreciate your kind attention to this....I sent the question at 2 a.m. so I guess that tells you that I'm concerned even with medical care we consider to be above average. Again, thank you...I sent in a question 20+ hours ago and no response....please follow up with the above asked question

Hi,

I am extremely sorry for delayed reply. My XXXXXXX had technical snag to reply.
I think wait and watch policy is better in your husband's case.
Rest, icing, compression and elevation all helps. I hope you already started it.
Make sure it doesn't get infected. The skin should not be punctured else infection is a high risk.
It takes a very long time than you expect to resolve the issue because of thinners and age.
Always keep the leg elevated.
