Question: My daughter had a fit in 2005 and CT scan was suggestive of calcified granuloma in right frontal lobe in 2005. She was put on zeptol for 5 years. Later in 2008 she experienced another fit . EEG in 2014 showed abnormal awake EEG record and evidence of probable irritative foci over both posterior head regions with secondary bilateral synchrony . background consists of 8-9 hz, 20-30 uv posterior dominant reactive to eye opening alpha activity symmetrically There is alpha dropout during sleep. electrode artefacts seen. occasional 1-2 hz spike and slow wave discharges are seen over both posterior head region, occasional generalized sharp waves are also seen, there is no activation with hyperventilation and photic stimulation. Kindly advise precautions and further course of treatment. Is this curable?

Detailed Answer:
Hi XXXXXXX
Thanks for writing in to us.

I have read through your query in detail.
A granuloma is small deposition of calcium in the brain after infection with neurocysticerci (dog tapeworm). When the worm dies, calcium gets deposited and this is called as granuloma. When this granuloma moves slightly at that time abnormal electric discharges take place in brain and this is basic cause of a fit or seizure.

In many patients the duration and frequency of a fit cannot be predicted. Sometimes there may be more than one granuloma and in these patients there can be fits which slowly increase in frequency if not under treatment.

The best way to prevent fits is by taking medications such as Carbamazepine (Zeptol) and the dose may be given as per the fits. A patient is also advised not to do high risk activities if uncontrolled fits are present. These activities include cooking on fire, swimming, driving a vehicle and operating heavy machinery. The reason is sudden fits may cause senselessness and the patient can harm themselves or those around.

If the fits dont come for for about 4-5 years then treatment may be discontinued after discussing with the doctor.

Further disease can be prevented by using hygiene practice when having meals and avoiding pork meat.

Hope your query is answered.
Do write back if you have any further doubts.

Regards,
Dr.Vivek
