Brief Answer:
First medical treatment is advised.

Detailed Answer:
Hi,

Thank you for your query.

1. A sphenoid sinus fungal ball is a non-invasive fungal growth in a sphenoid sinus.

2. If it is not symptomatic, or if this is an incidental finding, you can use hypertonic saline nasal washes with added steroid (budesonide), anti-allergic, steroid nasal sprays and steam inhalation for a few weeks. Anti-fungal medication is usually not advised as the fungus is technically outside the body.

3. if you can upload a few images of the CT Scan, it will be helpful.

4. Endoscopic surgery is a very safe and straight forward procedure with low recurrence rates. Balloon Sinuplasty is an option. The CT Scan helps in planning the safest approach. The operation is on the anterior wall of the sphenoid sinus, away from the optic nerves, carotid arteries, pituitary gland and skull base. Hence the risk of complications is very low.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
