Question: Big toe nail has a hard white plaque on it that can't be removed. Affects 2nd toe nail too. All other toe nail are normal. What is this and what remedies are available?

Based on the description and the image seen, you may have a fungal infection affecting the toe nails.

Fungal infections of the nail bed (tinea unguium) are treated by using oral medications.
They do not respond to topical anti fungals alone though the topical treatment can be added to the oral medication.

Examples of oral anti fungal medications are fluconazole (diflucan), itraconazole (sporanox) or terbinafine (lamisil).
These would normally require a prescription
