Question: Hello Doctor,
About 18 months ago I noticed a change in my left thumbnail.
It seems like the quick under the nail was receding as though it is dying off on one side. The nail itself feels hard and much less pliable than my other nails. I spent a couple of months regularly applying toe and nail fungus ointment but this didn’t make any difference at so I stopped. It is getting very slowly worse and there is now a color change to the about half the nail underneath the actual nail. It is only a slight color change but it looks like part of the spreading condition. As the quick recedes it has become thicker and has lifted the nail slightly at the edges. Now one fingernail and one toenail are both acting similarly but to a lesser degree. I am a 62 year old caucasian male.

Detailed Answer:
Hello
Thanks for the photograph.
It is looking like fungal infection.
Only topical creams will not help.
You need to take oral medications.
I usually prescribe fluconazole tablet weekly for 8 to 10 months and topical clotrimazole cream.
Hope this helps you.
Thank you
