Suggest Treatment For Frequent Urination And UTI

Question: Hi. Have diagnosed UTI. Started several weeks ago. Urgent care prescribed nitrofurantoin, although I told them I take it as prophylactic. Took meds for 2 days, developed terrible headache & nausea. They switched me to Levaquin (?). Took full course for 1 week. Thought UTI gone. Then a week later, symptoms back. Much worse at night. Went to my doc; she gave me cipro. Been on cipro for 4 days. Felt better until last night. Now pressure & urgency back like first symptoms. No blood. Not much burning. Just urgency & very frequent urination. Called my doc to ask about culture. They said it did show untreated UTI, but something about how I took AZO night before test. Told me to finish cipro (4 more days) and then do another urine test. Could this be something other than UTI? Why antibiotics not working? In past, I felt better w/in 24-48 hours.

Thanks for your query,based on the facts that you have posted it appears that you have Over Active Bladder Syndrome with incompletely cured UTI.

You have bee taking various antibiotics spread over a period of few weeks resulting into urine culture report being reported to be inconclusive .

Urgency and Frequency that you have are due to what is called as Over Active Bladder.

Start taking anti muscuranic drug like Darifenacin once daily .This will help to control urgency and frequency .(You will have dryness of the mouth and constipation which are known common side effect of this medication ).

Continue taking Ciprofloxacin and repeat your urine culture after a week.

Get prescription from your family Physician.

I seem to urinate more at night (not much during day), going several times before bed, and sometimes (not always) get up once a night. (I have one child and did notice that I would need to urinate during the night after giving birth, which was 8 years ago). But my recent & bothersome symptoms are as follows:

- I have noticed some leakage after urination, but only in the past few weeks.
- Then the UTI symptoms started: Pain, pressure and constant need to urinate, as well as the urgency feeling immediately following a trip to the bathroom. This symptom only started several weeks ago.

I went to urgent care for a these symptoms and was informed several days later that the culture was contaminated. However, the most recent culture from a visit to my GP last Friday shows bacteria and untreated UTI, which is why I am on cipro.

Why are antibiotics not working as quickly as I've experienced in the past? Is it common to have improvement for several days, followed by a flare up in symptoms again after several days on cipro? The fact that the culture did show bacteria leads me to believe that I DO have a UTI, but being on several antibiotics and having increased discomfort 4 days into the Cipro course is concerning me.

How are the UTI and the OAB related? Is the UTI causing the OAB, or are you suggesting that I have two separate problems? Will the OAB clear up once the UTI is gone? And if so, why would I take the anti-muscuranic? Are you suggesting that I take that as a continuous medication even after curing the UTI?

UTI has been not Eradicated .

Thanks for follow up.Your UTI has not been controlled because of the fact that the organisms may not be sensitive to Ciprofloxacin hence you have to repeat Urine culture to find out the organism and antibiotics to which they are sensitive to so that infection can be controlled with appropriate antibiotic as per culture report.

Over active bladder is a separate entity and can be associated with UTI .I would advise you to take Darifenacin only for 2 weeks till your UTI gets cured and later on observe for symptoms particularly urgency .If it persists you may need to take it for 2-3 months.
