Suggest Treatment For Frequent Hearing Loss While Having Tinnitus

Question: My question is to an ENT specialist who has dealt with Tinnitus problem due to SSNHL.

Hi Doctor,

I am XXXX & suffering with a high Frequency loss to my Left Ear only at 8000Hz which drops down to 70 decibel (may be called as (SSNHL)). Now for past 53 days I have consulted 2 ENT specialists and they gave various medicines. Like, Wysolone, Trental, Diavit Plus, Bilovas, Absolut 3G & Renerve plus injection once in a week. Currently, I am still continuing the same course of Diavit Plus, Bilovas, Absolut 3G & Renerve plus injection. There is a 50% improvement in my Tinnitus but not coming down from a certain point.

After repeated Audiograms from day 7 to Day 50. There is a small betterment but, the steep fall at 8KHz to 70 decibels remain same. I have high discomfort when exposed to.

1.     A telephone conversation while attending a conference call.
2.     Attending a XXXXXXX phone .
3.     High noisy zones
4.     Shopping malls
5.     Bit anxiety or while thinking to answer people questions.

Is there any Permanent cure? Or any hope of more betterment over a period of time.

1. Upload a few reference Audiograms.

2. The 8 kHz dip is a NIHL ( Noise Induced Hearing Loss ).

3. Some of the possible treatments are high dose steroids, TTS ( Trans Tympanic Steroids), HBOT (Hyperbaric Oxygen Therapy ), Tinnex (caroverine ) injection +"capsules, Hearing amplification / masking and TRT (Tinnitus Retraining Therapy).

4. Part of the discomfort is due to a phenomenon known as 'recruitment '. Loss of clarity of speech is seen in high frequency hearing loss. A programmable digital hearing aid should help.

Thanks Doctor,

I uploaded a file which has total case history along with Audiograms. Kindly have a review.

Further,

a)     I have no discomfort in listening to people. Only blockage somewhere inside the ear with some noise always.
b)     I feel more blockage if the noise is more which is one major point which I haven’t said earlier & the blockage remains less in a normal noise level zones.
c)     I have difficulty only in high noise prone areas as said above, What happens is the ringing in the left ear increases. But even than I can still hear all the communication.

Sir, If you suggest any of your known doctors name in XXXXXXX Or XXXXXXX I will visit him for further diagnosis with your reference.

1. This is NIHL (Noise Induced Hearing Loss). This is a narrow band hearing loss and does not fit the definition of ISSNHL. NIHL can be sudden. Is there any history of exposure to loud noise (occupational or otherwise)?

2. These symptoms of ear blockage, ear fullness and fluctuating tinnitus are consistent with NIHL.

3. The dizziness indicates more severity of the damage that extended to the (vestibular) balance system. It also raises the possibility of a viral labyrinthitis (any history of a cold?) or an early Meniere's Disease (though there is no low frequency hearing loss).

4. The treatment options remain the same as enumerated in my previous answer.

Thank you sir,

1. No, I am a software person work on computers in a low noise environment.
2. This happened in office where noise levels are very low.
3. Yes, I am having cold from a long time and is still there. ( two Drs. said it is due to pollution & do some steaming exercise. But there is no much improvement.)
4. I am leaving now to meet Dr. XXXXXXX Mohanty at Ramachandra Hospital porur which is close to my office.

1. There does not seem to be any occupational noise exposure as a source of the NIHL.

2. The cold should be treated. Pollution increases allergies. Steam inhalation will not affect NIHL.

3. Kindly let me know the progress. I have seen some improvement up to 3 months. Beyond 6 months, no treatment is effective.

Sir,

1.     They have done a Tinnitogram attached report, kindly review
2.     Regarding Blockage in Left ear, they said no cold is noticed
3.     They asked to continue same medication Bilovas thrice a day & Absolut 3G Twice a day for 3 months
4.     If no further improvement in Tiinitus they said let us do some therapy’s
5.     Regarding Frequency loss they haven’t said anything. It may Or may not recover, was the answer.

Please advise.

1. The Tinnitogram helps in localizing the frequency of the tinnitus. In your case it is matching the hearing loss, which carries a good prognosis.

2. Did the masking help suppress the tinnitus?

3. Continue the medication and revert back.

4. The NIHL is likely to be permanent.
