Suggest Treatment For Foot Pain While Being Treated For Jaundice

Question: My wife is suffering form illness fever since 4 months. She admitted and diagnosed as Jaundice (Bilirubin was 1.46) accordingly treated and Bilirubin is under control less than .8,within a months. But Fever being cyclic in 2nd half raise up to 101 degree F and comedown up to 97.4.
See visited to physician under his treatment followed by some test US, Scan declared as TB patient. yesterday test was done and observed ESR is up to 86.
See feting pain while folding right leg up in sleeping position.
Kindly suggest.

I had gone through your question and reports you have uploaded very diligently and I really appreciate your concern for your wife.

Her reports are suggestive of low hemoglobin, High ESR and low Albumin:Globulin ratio.
At the outset I would like further information to guide you better
Has she lost weight recently
Is there any history of cough or bloody sputum?
Her relevant medical history like diabetes or any other metabolic disease?
Any complaints of pain or swelling in her spine?

you need not to worry about jaundice as her reports are normal for serum bilirubin.

In my opinion you should go for montoux test, X ray chest and spine to establish a diagnosis
If she founds to be a confirm case of TB than she has to take a complete course of DOT therapy.

Meanwhile give her well balanced diet consisting of plenty of fresh fruits and vegetables
Multivitamin and iron supplements

She was in depression and drugs has been taken.
There is no weight loss. No cough like bloody.
She is no diabetes.
There is no complaint for swelling i spine but feeling pain some time.
She has been declared TB patient and take drug since three months.Recently test of report sent to you in which ESR shos 90.
She is suffering cyclic fever within 24 hours, between 97 to 101 since more than 4 months. While, bending right leg toward abdomen feeling smile pain at lower part of the abdomen. She has no will to take food.

Welcome again and thanks for all the details
She has been taking right medicines, Loss of appetite (no will to take food) might be related to drug induced gastritis.

She should avoid fatty fried spicy and junk food
She should take lots of fresh fruits, fruit juices and vegetables.

She should not stay empty stomach, ask her to have small and frequent meals
You need not to worry high ESR and Low Albumin : Globulin ratio is going in favour of Diagnosis of TB.

As she has been taking this treatment since last 3 months so I would advise her to visit her consulting doctor for physical examination and review the treatment, because she may not require to take akurit 4 for more than 3 months which can be switched over to just isoniazid and rifampicin drugs only.

Welcome again
High ESR, anaemia (low hemoglobin) and reversal of albumin globulin ratio (low albumin and high globulin) with bone pains warrants further testing to rule out multiple myeloma and related disorders.

I would advise you for plasma protein electrophoresis and X ray of the spine and affected leg to rule out this condition.
Urine should be tested for proteins or more specifically bence jons protein.
She should be tested for kidney function test like serum creatinine and blood urea.

As I advised you to revise her treatment for TB because she doesn't need to take all 4 antitubercular drugs instead she should take isoniazid and rifampicin to complete the course.
