Question: I have fibromyalgia, chronic fatigue, DDD, Ra,Ostio arthritis. A little over years ago XXXXXXX 1,13, Major back back fusion. I do as much as I can as for a home maker. but over the last few months Feels like Siatica is returning. My physican has been VERY compassionate trying different meds. I get lots of exersise as I live in a Quad Level home. pluss I did reserch and got mild exerses to do from XXXXXXX XXXXXXX because I was trying to do mild erobics with my husband, trying to support him, local VA found,he's diabetic. I admire him for trying so hard to take care of his issues, but think that I either pulled something loose,or the pins and screws in my spine triggered pinched nerves. It feels like a light switch turning off and on. The surgeon told me next morning I had, two vertabrae above where he worked that was deteorating and need to be carefull. 2 months later he moved to XXXXXXX from Mi. Ive had numerous assorted surgeries over the last 14 years and terrified that I have a failed back fusionand don't want to go through another. My lower back, tail bone, & spine bones hurt, and feel like nerves are being pinche and and burn. Muscle pain radiates from my butt down front thigh and ham strings including calfs. My little toe ,had no feeling after surgery, now the next 2 toes are numb. how much is fee?

Brief Answer:
Apply buprenorphine 5 mg transdermal patch locally.

Detailed Answer:
Hi

Since your lower back tail bone and spine bones is hurting and muscle pain radiates from your butt down to your front thigh and toes are numb i would advise you to apply buprenorphine 5 mg transdermal patch locally.

You can apply one patch to a dry, non-irritated, non hairy area of your skin on your upper body such as your chest, upper back, or the outside of your arms.

Leave it in place for seven days and then remove it and replace it with a new patch, but apply the replacement patch to a different area of skin on your upper body.

Avoid using the area from where you have removed the patch for at least three weeks. Continue to replace your patches once a week, on the same day of the week.

You need a doctors prescription for it.

If this is similar to fenetenal patch, it didn't help and had to get special tape to make it stick to my skin. Also any clue what causes night sweats, starts about 3am at the base of surgery and continues to spread soaking my hair. I bought a "cooling mattress pad it does help but unless I get out of bed and strip off pjs it will calm down. By the time I get up AM I'm cold. Also last fall was asked to come in to check thyroid levels, he asked if there was anything else going on? I told him I'de been dealing with diareah for about a month . I was loosing weight, 25 pounds. Dr suggested probiotics and yogert. Not working, I called back they said use pepto bismol. I ended up using Imodium AD,finally straitened out. With the exception of elevated BP,I wondered if I may have Adrenal problems and have approched him about this issue before, and after the fusion. Because I was taking percocet for sciatica due to spine colappsing he chose to wait till it was out of my system. I called to ask again this week but he is out of town. Thoughts about what to ask or expect?

Brief Answer:
Do a fasting blood sugar and thyroid stimulating hormone test

Detailed Answer:
Hi

Buprenorphine is not similar to fentany patch and it is 7 times more potent than fentanyl in reducing chronic pain.

Night sweats can be caused by low blood glucose level (hypoglycemia) and excessive levels of thyroid hormones (hyperthyroidism). I would get this checked when you meet the doctor again. A simple blood sugar level as well as thyroid stimulating hormone (TSH) will help us exclude diabetes and thyroid issues.

Adrenal problem such as pheochromocytoma which causes night sweats should also be ruled out. But it isn't my first possibility. You may also ask the doctor if he / she wants to perform blood and urine test for hormones such as catecholamines and metanephrines.

If the above mentioned tests come out as normal and considering history of weight loss and diarrhea, I would also advise for a bone marrow aspiration and biopsy test to rule out cancer such as lymphoma.

In other words this evaluation will be step wise process. Discuss about the process of evaluation with your doctor.

Until then I will advise you to sleep in removable layers of clothing, wear breathable natural fibers, avoid constrictive clothing, reduce consumption of warm beverages, reduce alcohol and caffeine intake and avoid spicy or very hot foods.
