Question: I am a 54 year old white female. For the past 2 weeks I have had several episodes of vaginal bleeding that just gushes out unexpectedly. This last time the vaginal bleeding had a real bad odor. In addition to these symptoms I have had flu like symptoms with some fever off and on for about a week. Also severe cramping and tenderness in the pelvic area. What do u think.

Brief Answer:
Malignancy needs to be ruled out

Detailed Answer:
Hi XXXX, I have gone through your question and understand your concerns.
First please tell whether you have attained your menopause and when.
This vaginal bleeding with foul smell can be due to many causes , of which the most important which needs to be ruled out are the malignancies in the postmenopausal female. Cervical, uterine or vaginal malignancies can cause irregular vaginal bleeding of these vaginal bleeding with sudden gush and foul smell are most common in the cervical malignancy.
Flu like symptoms like fever is common in malignancies.
Severe cramping and tenderness in the pelvic area can be due to associated infection in the genital organs.
If you have not attained menopause yet, then other causes of such bleeding episodes can be fibroid of the uterus, or severe infection in the genital organs, but malignancies need to be ruled out in this scenario also.
I would suggest you to consult an OBGYN specialist as soon as possible to get properly examined, investigated and treated accordingly.

I had not had a cycle for about 4 months then these symptoms began. I did have my Pap smear in Nov and it was normal. I do have type 2 diabetes and had a pelvic abscess in 2008. These symptoms strangely resemble how I felt back then

Brief Answer:
Possible pelvic abscess

Detailed Answer:
Hi XXXX, thanks for responding.
If you did not have your periods for 4 months and then had this bleeding episode, then a possibility of dysfunctional uterine bleeding is there, in which there is hormonal imbalance leading to prolonged amenorrhea of 3-4 months followed by heavy and prolonged bleeding, but there is no foul smelling discharge, pelvic pain or flu like symptoms in it.
Pelvic abscess is a likely possibility, but usually there is not so heavy vaginal bleeding in it.
Uterine cancer should also be ruled out.
I would suggest you to consult an OBGYN specialist as soon as possible to get properly examined, investigated (ultrasound and CT scan for pelvic abscess) and treated accordingly.Pelvic abscess needs to be drained as early as possible.
