Question: my mother is 53+ and she is having fever with chest pain.

Hello dear, thanks for your question on HCM.

I want to ask few questions first.

1. Sputum culture and sensitivity was done or not?
2. Have you consulted pulmonologist?

In my opinion she is having resistant pneumonia or Tuberculosis.

As she was improving with antibiotics earlier (total count was decreasing) and again she is showing increasing total counts with symptoms like fever ,chills And chest pain.

So we need to find the organism causing x ray infiltration (pneumonia).

For this sputum culture and sputum for AFB is needed. If she is not having sputum production than Bronchoscopy and XXXXXXX ( Broncho Alveolar Lavage ) analysis will be the most suitable test.

Another possibility is tb. As in old age tb can present as pneumonia without classical cavitation on chest x ray.

So better to consult pulmonologist and get done Bronchoscopy and XXXXXXX analysis to find out the causative organism.
