Question: I thought I had forgotten to take my effexor this morning, and I took it a few hours ago. I feel tired and a little shaky. I've had mild episodes of serotonin syndrome multiple times before, and am quite prone to it. if I was mistaken and have now taken 450mg of effexor today, should I be worried?

Brief Answer:
You need not to worry much

Detailed Answer:
Hello,
Thanks for writing at healthcaremagic.

I am aware of your risk for serotonin syndrome and like to opine following things.

1. Since the increased dose of medicine was similar one (venlafaxine only) the risk of serotonin syndrome is very low in compare to two or more medicines with serotonergic properties

2. Feeling tired and shaky is not classical symptoms of serotonin syndrome although it may occur in very mild serotonin syndrome.

I request you to do following things to ameliorate the symptoms
1. Take rest
2. Take plenty of water
3. Stay in calm and non disturbing area
4. Take a low dose of xanax (alprazolam 0.25 mg) or clonotril (clonazepam 0.5 mg).
5. Skip the next dose
6. Avoid any substance of abuse particularly opioids

I hope these simple but effective measure will sufficient for you at this stage.

In case shakiness increases or you develop any autonomic disturbances (hypotension, tachycardia, sweating, rapid respiration), neuromuscular abnormalities (abnormal movements, rigidity), or Fever please visist nearby emergency.

I must assure you that current symptoms can be managed at home and you do not need to worry about them.

Hope I am able to answer your concerns.
