Brief Answer:
Decongestant tablet and spray may help

Detailed Answer:
Hello,
Welcome and thanks for your query.
Appreciate your concern for your wife's distressing problem. The facial pain at high altitude or in the flight is because of the imbalance in the pressure between the air in the sinuses and the air outside. This is because of the block of the opening of the sinuses to nasal cavity. This is because of mucosal swelling.
She can get relief by using a nasal decongestant spray or drops before the flight and use of decongestant tablets like clarinase and pain killers.
If these are not sufficient, she would require Functional Endoscopic Surgery (FES ) to produce drainage and aeration of the sinuses.
Do get back to me if you have more queries.

Dear Dr. Noble

Thanks a lot for our answer.
She already taken many time the nasal decongestant spray before going for flight and even the local doctors suggest her to take this spray at the interval of half an hour during the flight and also taken pain killer, But no success, she bear too much pain no the face.
That is correct her face becomes frozen and she cant eat any thing during the pain, as soon as the plain is about to land, she feel lesser pain and slowly slowly calm down.
Can you please suggest me another remedies.
wait for your reply
thanks XXXXXXX

Brief Answer:
FES is the permanent solution

Detailed Answer:
Hello Mr. XXXXXXX
She can take a decongestant tablet like clarinase twice a day for a couple of days prior to and during the flight.
The lasting solution is to see a good ENT surgeon and have FES to enlarge the connection between the nose and the sinuses so that air can move freely in and out. She will not have the problem and be pain free then.
Best wishes,
