Question: What are the best questions to ask your urologist about ED and how to treat and reduce its occurrence. I am 69 and have tried the Calais, Viagra route and the penile shots to cause erection. All have worked to some degree but it seems ED just keeps creeping back. Alas, I have about given up hope of finding a useful treatment.

Thanks for your query,based on the facts that you have posted it appears that you are facing problem of Erectile Dysfunction which does not get resolved even after taking medications like Viagra ,Cialis ,or Intra Corporeal Injections of Alprostadil .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Are you diabetic ?
2) Do you have hypertension ?

The basic cause of ED at elderly age is reduced blood flow to erectile tissue in penis.
.
This is the result of generalized atherosclerosis of blood vessels that also involves Penile Artery which supply arterial blood o erectile tissue in penis to achieve desired hard erection .This is supposed to be natural and normal process with advancing age .

The resulting narrowing of these blood vessels leads to reduced blood flow to erectile tissue in penis causing ED.

You need to get this confirmed by a test called Color Doppler Ultrasound Scanning of penis .If based on this test results your ED is turned out to be Vasculogenic in origin there is hardly medical treatment that can resolve your problem of Ed .

In such situation you will have to explore possibility of putting Penile Prosthesis (Implant ) so that you can carry on with your sexual activities and satisfy your female partner.
