Question: Thank you .You're very kind to be of help. I am 69 years old, male , Caucasian, in good-excellent health. I have had a problem with erectile dysfunction for the last few years. I consult a urologist but I don't respond to Cialis and I dread penile injections. I also think depression may be a strong contributing factor.

Erectile dysfunction (ED) can be due to many reasons among which, medications, chronic illness, poor blood flow to the penis, alcohol, or simply being too tired.

It is a common problem once males get older, but being common does not mean it is ok. It is important for the general well-being of males.

It is not easy to diagnose what is the reason of ED. Proper history has to be taken, which drugs do you use, anxiety and depression can play a role, do you have morning erection, any occausion of erection, does it start and then stops (maintaining erection for example is the problem or getting one), your doctor should do CBC, blood sugar level, hormonal work up (thyroid, testosterone levels check), study blood flow to penis (duplex ultrasound), urinalysis, prostate test (PSA). Blood pressure, EEG, cardiac output also needs to be tested.

Meanwhile, you need to adjust your lifestyle, get more exercise, eat healthy food; spicy food can help in erection. Some natural supplementation can help too, but you need to make sure all supplements are given under medical supervision. If you smoke and drink alcohol you need to stop.

If you think you have depression, stress or anxiety, you need to consult a psychiatrist. Since such problems can lead to ED, and anti-depression drugs like serotonin reuptake inhibitors can help in cases of ED.
