Question: Hello, I have a question concerning ED. I am 63 years old, I have tried Cialis (20mg) and Viagra (50mg) and they do not work. Should I increase the Viagra dose to 100mg?
The meds I am taking are: Metformin 100mg/day, Lipitor 10mg, Prednisone (went down from 30mg to now 6mg because of PMR), Uroxatral 10mg/day, LIsinopril 2.5mg/day.

Hello

I am Dr.Omer and i am here to help you with your query.

ED ( erectile dysfunction ) has a wide range of causes.You may have the followings:

1.Poor diabetes control.Check your blood sugar as well as HBA1c to see how good your diabetes are controlled.Prednisone it-self causes diabetes since you are using very low dose of metformin (100 mg).

2.PMR (polymyalgia rheumatica) meaning fatigue and pain in shoulders ,hips can also cause vasculitis of penile vessels meaning inflammation of blood vessels so ANA level with CRP/ESR test will show us your severity of PMR and its penile involvement.

3.Stop smoking if you do and a good sleep can help you alot to over-come this problem.

4.Alfuzocin is a drug for BPH (prostate) , its common side effect is ED , so discussing this with your doctor can eliminate one of the cause of ED.Besides alfuzocin has the maximum drug interaction with sildenafil causing low blood pressure with dizzy spells.Increasing the dose of sildenafil to 100 mg per day is safe , but you need to discuss it with your doctor to take out alfuzocin, or you can stop taking it for a day or two.The best time for viagra to work is from 1 hr after taking it to less than 2 hrs.

5. Lisinopril also causes some leevel of ED , discussing with you doctor about changing it to losartan ( a sistern drug ) will be helpful.

6.Finally low mood can also cause ED and it can be exaggerated by any drug.So if any symptom present due to diabetes , ED , anxiety , insomnia , polypharmacy , or a by-product of diabetes.A counselling with Psychiatrist will be helpful.

Thanks Dr. Omer, this is very helpful.
I forgot to mention that I was taking Insulin (28 units/day) to control the diabetes caused by Prednisone. Before the PMR events which started last XXXXXXX I had a terrific blood sugar and A1C control (5.6%) for ten years.
Question: should I replace the uroxatral with Cialis 5mg/daily? and shall I lower the Lisinopril?
Are you allowed to prescribe?

So you are taking insulin as well as metformin , just check your blood sugar and HBA1c regularly.Prednisone does cause disturbance of diabetes control.Just adjust your insulin dose according to your blood sugar level.Strict blood control is required.

HBA1c of 5.6% shows good blood sugar control.

Cialis is a sistern drug of viagra.You cannot and should not take cialis and viagra together as it will cause dizziness and your ED will be exaggerated instead of getting better.Take either of the two drugs , viagra is better and time tested drug than cialis for diabetes and prostate problem.The benefit of cialis is has long duration of action for 24-36 hrs and can be taken as 10 mg , 60 min before sexual activity or 2.5-5 mg every day.

Alfuzosin is a alpha blocker and you cannot subsitute it with cialis as both have different mechanism of action and alfuzocin is used for prostate problem and cialis is taken for ED.You can stop taking it for a week and you will see a better effect of viagra or cialis on you.

Lisinopril is kidney protective drug from diabetes.You can subsitute it with losartan rather than decreasing its dose.

No i am not allowed to prescribe any drug on this site but i can assist you what you should do and tell your doctor so that he/she changes all these medicines.

Lipitor has a side effect of chronic prostatitis which causes ED in return.I would recommend to change it with parvostatin which has least prostate effect.
