Suggest Treatment For Dry Scalp With Itching
Question: My wife has a problem of dry scalp with itching. She gets itching at few concentrated area on the scalp with white dry skin. We have been recommended solution named "Topisal" but problem reoccurs when after sometime again.
She also has hair falling problem. She is undergoing treatment for hypothyroidism with does of 1 tablet of 50 mg per day. Her T3, T4, TSH are within limits. Please advise.

I have read your query.
I understand your concern.
As per details, I would like to put following differentials for her dry looking itchy scalp-
1) Scalp psoriasis - it is characterised by well defined areas of scales dposited in scalp on background of erythema. It may or may not be associated with itch.
In such cases, I prefer to advise topical steroid like clobetasol in lotion form to my patients. Anti dandruff shampoo containing ketoconazole is also helpful.
2) Seborrhoeic capitis - it is characterised by ill defined areas of fine scales deposition almost over the whole scalp.
For this anti dandruff shampoo and topical steroid is the treatment of choice.
3) Tinea capitis- it is fungal infection of skin involving the scalp associated with itch.
It may be of black dot type or grey patch variety. The hair roots get weak and are easily broken on pulling the hait
For this, oral and topical antifungals are advised.
The first two above mentioned conditions have high chances of recurrence but the third condition is completely curable.
Wood' lamp examination is helpful making diagnosis.
