Suggest Treatment For Drug Resistant Pulmonary Tuberculosis

Question: hi sir my father had diabetic since three years for that using insulin and maintaining diet also but eight months back my father got cough continuously and chest pain and spitting blood also then we consulted pulmonologist ...find TB Positive in sputum AFB test then prescribed some medicine R CINEX 450 MG AND one more tablet for night with name of XXXXXXX ....full name i don't know ....so my father used this tablet for 8 months and after using this tablet cured cough and spitting blood also ...now he is fine since six months till date and while using medicine we consulted doctor five times always doctor checking x ray and some blood test also done and used to say continue this tablets ....when our last visit after using tablets 8 months doctor said go for branchoscopy test then we went for branchoscopy and got some reports in five days doctor said everything is fine no problems in the reports and said some reports will come in weekly wise for five weeks so in first week report we got negative but in second week report we got positive then we consulted doctor again ...docotr said go for 13 drug test then will write the tablets again but that drug test cost said 12000/- rupees ....so pls tell me can i go for that check-up because my father every thing is fine ..no pain and no blood spitting and no cough...not getting sputum for AFB ...so pls suggest me should i go for that test to use tablet again or else pls let me know any tablet has to use my father ...OR any thing.......abt weight 6 months back my father weight 52 . now he is 63 weight

It seems your father with diabetes has drug resistant pulmonary tuberculosis since even after 08 months of first-line AKT with R-cinex,likely Combutol and Benadone (Vit B6),his bronchoscopy sample has grown AFB.Hence your doctor /pulmonologist has asked for drug sensitivity tests. However in stead of 13 drug profile,at first one can go for rather a short list of second-line drug(especially the fluoroquinolones and the injectable drugs) profile and it will cost less.Results are obtained rapidly by direct susceptibility testing on liquid medium, with an average reporting time of 3 weeks. Drugs will be selected as per sensitivity profile .

sir ,now he is everything is fine so pls let me know ..can i go for that drug sensitivity chekup or not and that docotor is telling 12000/- rupees for that test already we spent 40000/- rupees till now

Hi,

There is no doubt that your father has pulmonary tuberculosis and resistant to present drugs. Hence you need to go for sensitivity tests. MDR diagnosis & treatment is expensive but there is no other alternative,as it will be fatal ,if left untreated and also will spread to other family members with disastrous consequences.
